0002155i81 2002-05-23 Production Wells Violations Noted While investigating a gas odor complaint (CASE 02-024) it was discovered that the Anderson well is producing into a tank battery that is labeled Brunswick Twp Trustees #1 well. The separator is no longer used and the gas appears to be vented through a stock tank. The tank battery consists of three 140 bbl stock tanks and one 6,000 gallon former underground storage tank, now aboveground, for brine storage. A straw covered kill area inside of the dike indicated either a brine release or a stock tank overfill. The drain on the eastern stock tank was also leaking. The Anderson well is equipped with a pumping unit, tubing and rods. According to Mr. Williams, the stuffing box recently leaked and the spill area was covered with straw. The well is reportedly pumping 24-7. Flow line and sales line for this well are all aboveground steel.
0010179353 2002-06-05 Production Wells No Violations The oil spill around the wellhead has been remediated. The impacted soil and straw was removed and the area was regraveled. The site will be monitored for any reoccurrence. The tank battery violation has not been fully addressed. The brine leak from the stock tank has been plugged and corrected but the impacted area inside the dike still needs to be cleaned. Requested that Mr. Williams notify us when the dike work is being done so that the condition of the brine tank can be evaluated. The identification on the tank battery also still needs to be corrected to identify the Anderson well.

Stuffing box has minor leak. Pumpjack motor is running on natural gas, 0% lel in air. Located a popoff valve in gas line to allow gas to escape periodically in park north of well. Called Dennis Williams to discontinue gas blowing in park and set up flare. Called Mr. Schulte and advised him the gas will be flared in approximatly one week. Cloudy, 75 degrees.

The tank battery is used by multiple wells; identification is posted for all wells utilizing it, but must be updated with the current operators information. The well is equipped with a pump jack with an electric motor located within an unlocked metal fence. Well identification is posted at the wellhead. Research into the production history showed that no oil or gas has been produced since 2008, only brine was produced in 2009, and no production has been filed since 2011, therefore the well must be plugged.

The tank battery consists of 2-100 bbl steel tanks, 1-50 bbl steel tank, and 1-vertical separator located within a partially collapsed earthen dike. Well identification is posted at the tank battery, but it reflects a previous operator and must be updated. The well is equipped with a pump jack with a gas-powered motor located within a locked, but partially collapsed metal fence. Well identification is clearly displayed at the wellhead. Well identification is posted at the wellhead as required. Received 2014 production data from Landmark (which was turned in to Steve Opritza). Production data shows that the well did not produce in either 2013 or 2014. Will discuss plan forward with Landmark 4.
