0319313781 2018-05-08 Plug / Plug Back No Violations Buxton Well Service was rigged up on location with a double pole service rig. The 1.5 inch diameter tubing was removed from hole. Formations Cementing arrived on location and set a Clinton bottom hole plug from 5882 feet to 5482 feet within the 4.5 inch casing a crossed the perforated section. The plug was set through 4.5 inch casing, consisted of 30 sacks of class A cement weighing 15.6 pounds per gallon, and was displaced with 87.2 barrels were used for displacement. The 4.5 inch diameter casing was shut in overnight allowing the cement plug time to cure.
0396024957 2018-06-06 Final Restoration No Violations The purpose of my inspection was a status check on a final restoration. During my inspection the production equipment had been removed and the pit had been closed. The lease road and wellsite had been graded with vegetation established. The location has met final restoration requirements written within 1509.072 (B).
0726302098 2018-05-09 Plug / Plug Back No Violations Appalachian Well Surveys rigged up a casing cutter and tripped in the 4.5 inch diameter casing and shot the casing at 4855 feet in depth. The wireline tripped out of hole. The rigged pulled up on the 4.5 inch diameter casing and pulled 10,000 pounds over string weight. The casing was not free. Appalachian rigged up a casing cutter and tripped in the 4.5 inch casing again and shot at 4400 feet. The wireline was tripped from hole and the rigged confirmed that the casing was free. Five joints of 4.5 inch casing were removed from hole. The bottom of the 4.5 inch casing was at 4189. Appalachian left location and Formation cementing arrived. Formation cementing pumped a Big Lime plug from 4189 feet to 3989 feet. The plug was set through 4.5 inch casing, consisted of 60 sacks of class A cement weighing 15.6 pounds per gallon, and was displaced with 63.4 barrels were used for displacement. Buxton Well Service tripped 63 joints of 4.5 inch casing from hole leaving the bottom of the 4.5 inch casing at 1536. Formation Cementing pumped a bottom of surface casing plug from 1546 feet to 1346 feet. The plug was set through 4.5 inch casing, consisted of 60 sacks of class A cement weighing 15.6 pounds per gallon, and was displaced with 21.4 barrels were used for displacement. The 4.5 inch diameter production casing was removed from hole. The fluid level was roughly 100 foot down. Formation cementing topped the 8.625 inch diameter casing off with a 100 foot to surface class A cement plug.

1701334237 2015-08-11 Production Wells No Violations On the day of my inspection accompanied by Engineering Supervisor Mark Atkinson, Engineer Tech Wayne Cottle and Jerry Smith, I witnessed the production equipment included a 100bbl tank, a horizontal separator, a 2 inch sales meter with a 1 inch orifice and a chart recorder. The well was equipped with 8.625 inch casing, 4.5 inch casing, 1.5 inch tubing, a plunger lift system and domestic gas service. Identification was present and production records were current.
