0893666527 2018-07-16 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design, 5.5-inch diameter production casing that appeared to be open flowing. Annular pressure monitoring was in place on both the 13.325 inch diameter surface casing and the 9.625 inch diameter deep intermediate casing, pressures were monitored with transducers with no ability to read pressures. Both casings had bleed down lines ran to the production fluid tanks, at the time of my inspection the 13.375-inch diameter casing was open flowing into tanks. Production equipment included a Sand Trap, and Gas Processing Unit. The wells were still set up with flow back equipment. Sand traps were running to 2- 500-barrel horizontal frack tanks. There were also 6- 400-barrel production fluid tanks inside containment. Production fluid tanks were all shared with 34013209950000, 34013210000000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
1258495920 2015-10-15 Casing String Witnessed No Violations On this day Express Energy had drilled the 32 open hole to total depth of 100. The 30 conductor pipe was ran and cemented. Express Energy was on site to cement the casing with 275 Sacks of Class A cement. Cement to surface was established. See cement ticket for details.
1364859077 2015-10-19 Drill / Deepen / Reopen No Violations A lost circulation zone with unconsolidated material was encountered from 524 to 575. Drilling continued until 30 into a good solid formation. At this point the hole was cleaned and First Class pulled out of the hole to run 18.625 casing.
1445337684 2015-10-20 Casing String Witnessed No Violations On this day First Class Energy had reached a total depth of 634. The 18.625 casing was ran into a 26 open hole to a depth of 608.C & J services was on site to cement the casing with sacks of Class A cement. A balance job was preformed from the shoe to the lost circulation zone. A basket was set at 500 and the annulus was filled from the basket up with cement. Cement to surface was established. 1557258702 2019-03-25 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with erosion control and vegetation established in the areas affected by the well pad construction. Signage is posted at the well pad lease road entrance and at each of the 3 well heads present on location. The lease road and well pad surface is stone construction and in good repair. There is no perimeter berm on the well pad. Production equipment includes (6) 400 barrel steel tanks situated inside a polymer lined steel containment dike area long with (3) gas processing units. The wells are selling off the 5.5 diameter production casing through emergency shut down devices and choke manifolds. Additionally (4) wells are proposed on this location.
