0557259948 | 2019-02-07 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found The well site meets ORC 1509.072 with vegetation established and erosion control in place. The well site boundary sedimentation control has been removed. The well pad and lease road surface is stone construction and in good repair. The well pad perimeter is protected by an earthen berm, there is also an 8 chain link fence present around the well pad. There are 5 wells present on the pad. Identification is posted on each well and production tank along with the entry point of the lease road and well pad. The well is equipped with a lower A section and upper B section with 2 diameter pipes present attached to the service ports of both sections for the purpose of annular pressure monitoring. The riser pipes are equipped with valves and both electronic transducers and manual gauges. The pressure on the well head A section is 504 pounds pressure and the pressure on the B section is 1,027 pounds pressure. The 5.5 diameter production casing well head flow cross pressure is 586 pounds pressure and the pressure on the production flow cross section is 1,000 pounds pressure. The master valves have been removed and a production tree has been installed on the #2 and # 10 wells on the pad. There are 5 gas processing units on location and (2) production fluid storage tanks situated inside a polymer lined steel containment dike area. There is a Mark West mid stream pipeline service point at the left rear corner of the well pad. Emergency shut down devices are present at each critical area of the well pad including the entry point of the location. |

22:30 Schlumberger pump truck showed up. Rigging up and will start pressure testing and proceeding with the job. Broke circulation and placed the gel pre-flush and then the water spacer. The cement slurry was started and pumped 338 total barrels of slurry. There was 1 sample taken and weighed at 15.6 lbs. per gallon. The pump truck shut down and released the rubber wiper plug and started the displacement. There was a sample of cement slurry taken at the return side and weighed. The lift pressure was 698 psi and bumped the plug at 500 psi over. The pressure was held at 1155 psi for 5 minutes and released back and the floats and check held. There were 64 barrels of cement slurry back to surface. The displacement was at 214 barrels of water. There was good circulation and there was total of 1600 sacks of Class A cement pumped with 200 lbs. of cello flake or .125 lb. per sack, low temp dispersant additive and Flocculation additive added. | |
