0080268636 2017-06-14 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1765 feet. Previous casing set and cemented was 18.625 inch diameter mine string to a depth of 360 feet. Wyoming casing ran 1758 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot. Casing was centralized with 14 centralizers. Shoe track is 44 feet long. O-tex was onsite with 1470 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 257 barrels of fresh water. 35 barrels of gel pumped ahead of cement and 10 barrel of fresh water between gel and cement. 70 barrels of cement returned to surface, and floats held with 1.5 barrels back. Mix water has pH of 7 and 46 ppm chlorides.
0865279395 2018-11-27 UrbanProduction Wells No Violations Status check inspection found (3) wells present on the well pad 2 are in production one does not have the drilling process complete. The said well is producing off the 5.5 diameter production casing through 2 primary 7.0625 diameter master valves. The 5.5 casing pressure is 1,200 pounds per square inch (PSI). There lower well head assembly consists of a lower A section and upper B section, attached to the A&B section outlet ports is 2 riser pipes equipped with valves and gauges for the purpose of annular pressure monitoring. The A section riser pipe gauge reads 350 pounds pressure, this is the pressure on the internal of the 13.375diameter casing string. The B section riser pipe gauge reads 1,500 pounds pressure, this is the internal pressure on the 9.625 diameter casing string. The well head production tree consists of 2 flow lines both of which are equipped with (2) emergency shut down hydraulic valves and electronic choke valves. Each flow line has a vertical separator and one gas processing unit per well. Also present is a glycol heater. There are (2) 500 barrel production tanks present on location, the tanks are situated inside a polymer lined steel containment dike area. The well pad perimeter is protected by an earthen berm that is equipped with an 8 chain link fence. The well pad entrance does have a gate that is equipped with a combination lock.
1496678113 2017-05-31 Casing String Witnessed No Violations On the day of my inspection I witnessed precision rat hole had augured a 36 inch diameter hole to 94 feet. 96 feet of 30 inch diameter conductor casing was ran. O-tex was onsite with 310 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.20. 50 percent excess ran. No returns to surface. Cement came within 20 feet of surface and then fell back down hole. I left location and returned to this pad for top of job on the same evening. The 1H well took the cement that was supposed to go into this well and no cement was pumped into this hole on the evening on 5/31/17. On the morning on 6/1/17, after returning to location for third time, 307 sacks of class H (2%CaCl, 3% gypsum, 15.6 ppm 1.2 yield) cement were pumped down the back side with no returns to surface. I again left location and returned the afternoon of 6/1/17 to witness the pumping of 200 sacks of class H cement down the backside. No returns to surface. These class H blends are referred to as Henderson blend in the OTEX paperwork. Inspector Randy Akins returned to location on 6/2/17 to watch the pumping of an additional 240 sacks of class H with gypsum. Otex paperwork refers to this blend as Dan Ryen and it has a weight of 14.2 ppg and yield of 1.67. 1057 total sacks of cement pumped over 5 separate inspections. Well was spudded on 5/30/2017.
1497297025 2017-06-12 Casing String Witnessed No Violations On the day of my inspection I witnessed the mine string cement job. Falcon 26 drilled a 24 inch diameter open hole to a depth of 367 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 96 feet. Wyoming Casing ran 361 feet of 18.625 inch diameter casing, j-55, 87.5 lbs/foot. Shoe track is 24.87 feet long. Cement Baskets were set at 294 feet. O-tex is onsite with 750 sacks of Class A cement with a yield of 1.2. 70% excess ran in open hole. Cement displaced with 103 barrels of fresh water. 30 barrels of gel pumped ahead of cement. Cement weighed 15.6 ppg on pressurized mudscale, truck read 15.9 ppg. 40 barrels of cement returned to surface, and floats held with .5 barrels back. Mix water has pH of 7, 46 ppm chlorides, and 73 degrees Fahrenheit. Coal was encountered at 315-325 feet. All depths, except conductor, are from kelly bushing which is 12 feet above ground level.
2019425800 2018-08-16 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design with 5.5-inch diameter production casing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 1125 pounds per square inch, the 13.375-inch diameter surface casing gauge read 285 pounds per square inch. Production equipment included a 2 Sand Traps, a line heater, and gas processing unit. There are two 500-barrel production fluid tanks shared with the other well on the pad. There were multiple Emergency Shut Down devices on location, Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
