| 1354809452 | 2012-12-05 | Production Wells | Violations Noted | Pryor to my inspection I received a phone call from Mr. Dean Shriver requesting me to do a inspection a well I.D.#34-059-2-3178-0000, after doing my inspection I found the well has been produced through the two inch tubing and the packer, the valves on the tubing were open, the valve on the production line was closed, there is (1) two hundred ten barrel production tank/ separator/ chart setting inside a containment dike, the date on the chart says 01/22/2010. I spoke with Allen Tarleton with Enervest about the condition of the well and he told me the packer was leaking. The divisions records for the year of 2010 and 2011 doesnt meet state standards. The requirements will be to plug/ produce or place in temporary inactive status, failure to comply will result in forfeiture of bond. |
