He said the 4 1/2 casing was sticking out of the well approximatly 50 and debris is laying all over well location. He also advised the well was on fire at the time it blew up. At 2:25 pm on 11/12/13 I arrived on this location with Mr Pivonka and the landowner , we walked back to the wellsite and tank battery , the well is in production and has no pipe sticking out. Found no debris on site. I then asked the landowner who lives on property where this well is located about it blowing up and he advised it never happened. I then asked Mr Pivonka why he said this well blew up and he advised it was because he thought it did. |

1950885034 | 2016-06-14 | Production Wells | No Violations | On the day of my inspection I witnessed the well was equipped with 8.625 inch casing, 4.5 inch casing, 1.5 inch tubing and plunger lift system with all valves in the closed position. As I inspected the wellhead I was approached by the land owner who stated I did not have permission to be on the property. I stated that I was the State inspector and ORC 1509.03 gives me the right to be on the property and that I apologized for not stopping at the residence prior to entry. The land owner then threatened bodily harm to me if I returned and told me to do what I needed to do and get out. I completed inspection of the production equipment which included a 100bbl tank, a 50bbl polymer tank, a horizontal separator, a 2 inch salesmeter with a 1 inch orifice and a chart recorder. Identification was present and production records were current with zero production reported. Upon leaving the landowner stated that the gate would be locked in the future and he apologized for his actions. I placed call to Bill Tipka and requested a plan of action for the well. Mr. Tipka stated that he has issues with the land owner and gaining access to the well also he stated that he would get back to me with their plans. Brian Tipka called me and discussed the well, stating it is capable of production however he does not feel comfortable sending a contractor to the well due to the issues with access. He then stated that they are trying to work out an agreement and gain access from Speck Road. I stated that if the well cannot be put back in production I would be sending a violation and citing ORC 1509.12B. Mr. Tipka stated he would get back to me whether they will be able to gain a different access point. |
