Operator reports started well up at 7-8 AM, had been down for couple months, and tank leaked when oil went in tank. Luckily, oil had been picked up prior so only tank bottom in tank. Feel lost 5-6 bbl, went over a poor dike and downhill into Little Muskinghum Rv. Travelled about 1000 shen caught in a bend of the river, quickly put booms out, pads and vac truck out to suck up the oil. River almost clean when I left site. Will write NOV for pollution and contamination, informed OEPA on 7/25/10 - will call back with details on 7/26.
1310558556 2011-07-11 Production Wells No Violations Inspection this day found well with jack in production, location has been cleaned up from spill. There is large pit at tank but tadpoles, frogs say it is fresh. Good IDs on tank, good dike, well has been added to bond.
1510216826 2008-02-01 Production Wells No Violations Inspection this day found well producing with jack. Could not hear oil or water running in tank, had obviously pumped off and well still running 2 hrs. later. Tank has dike, pit for draining the Exempt Miss. water into, IDs on tank say owner is Double D. MIKE - add to bond.
1848724768 2010-07-24 Production Wells Violations Noted Clean up oil spilled to ODNR/OEPA satifaction. Replace tank and build sufficient dike. Add well to your bond, turn in annual production. 7/11//2011 - RESOLVED - well has good IDs, producing with new tank set, well added to bond. This new owner not responsible for past production. Spill has been cleaned up.
1875204258 2010-08-11 Production Wells No Violations Oil spill now completely cleaned up. All debris picked up, well still does not have jack reset, tank still needs fixed or replaced and well not producing yet. Location has been dozer level and reseeded with grass starting to come up. Owner at time of spill, Darrell Yoder, is selling well to Rob Armstrong, in process of sending paperwork to Columbus to add. No annual production submitted yet. Will not resolve NOV until all issues corrected.
