0091633396 2015-02-18 Casing String Witnessed No Violations I was on the Kiko # 3 - H location to witness the placement and cementing of the 13.375 surface casing. Sidewinder Drilling Rig #63 drilled a 17.5 borehole to 1056 KB. The surface hole was finished on freshwater fluid due to the influx of water at 744 KB. Kerzey casing crew placed 25 joints of 13.375, 54.5 lbs./ft., J-55, ST&C casing to 1020KB. Casing hardware included a float shoe, a float collar and five bow spring centralizers. Universal Well Services ( UWS ) established circulation with 28 barrels of water followed by a 50 barrels gelled spacer and 5 barrels of water. UWS mixed and circulated a 174 barrels cement slurry ( Type 1, 3% CaCl, 0.25 lbs./sk. celloflake ) and displaced it back to surface. Displacement was achieved using a top plug and 149 barrels of water. Cement returns observe numbered 25 barrels. After the plug was bumped to 1000 psi. and held for five minutes without any decline the pressure was released. .5 barrel of flow back occurred and stopped. All floats held. A walk around tour reviewing the solids handling area, SOBM tanks, mud pumps and cellar did not reveal any violations.
1052060273 2015-02-24 Drill / Deepen / Reopen No Violations I inspected the drilling activities at the Kiko # 3-H on 2/24/2015. Sidewinder Drilling Rig # 63 was mist drilling a 8,75 borehole at 5096 KB. This is the third ( 3) and last scheduled hole on this pad. A walking tour inspection looked at solids control, SOBM tanks, air compressors, boosters and cellar. No violations were identified during this inspection.
1151360913 2018-06-27 Production Wells No Violations I conducted a status check of the well and production pad. The pad is equipped with three producing wells (permit numbers 22634, 22635, & 22637). This well is equipped with plunger-lift configuration. The surface-intermediate annulus was equipped with a pressure-relief valve and a pressure gauge which displayed a reading of zero psi. Proper identification was posted. The tanks and production equipment were contained inside a steel lined steel dike. Less than an inch of standing water was present in areas inside the tank battery. The access road was in good condition. Erosional controls were in place and appeared functioning.

Equipment on the pad consisted of the following: Nine 400 bbl production tanks; six labelled as containing condensate and three as containing produced water Three gas processing units with vertical separators Two combustor units Three vapor recovery units One oil stabilization unit and tower

1354758045 2018-04-06 Production Wells No Violations I performed a status check on the above location. At the time of my inspection there was a rig on location swabbing the 2-3/8 tubing. All wells were in production while this operation was in progress. ID signs are posted. There was no noted violations at this time.
1416320096 2014-11-13 Casing String Witnessed No Violations I was on the Kiko # 3 - H location to witness the placement and cementing of the 20 conductor casing. H A D Inc. drilled a 22.25 borehole to 120 GL with an Air Rotary drilling rig. H A D Inc. placed 116 ( 6 joints ) of 20, 88 lbs. / foot, welded joint casing into the borehole.The conductor casing had circulation windows cut into the casing 5 ( @ 111 ) from the bottom of the casing for the placement of the cement slurry. Universal Well Services ( UWS ) connected to the casing and established circulation with 5 barrels of fresh water. Mix water temperature was 38 degrees and pH was 6. UWS mixed and circulated the cement slurry back to surface. The cement slurry consisted of 100 sacks of Type 1 with 3% CaCl2 ( blended ).I observed 4 barrels of cement slurry returned back to surface. Cement fell approximately 5 and will be topped off, if required, when the cellar is constructed. No violations were identified during this inspection.
1934919025 2015-03-08 Casing String Witnessed No Violations I was on the Kiko # 3H location to witness the latter part of the cementing of the cementing of the 5.5 production casing. Kerzey Casing Service ran 302 joints of 5.5, 20 lb./ft casing to 12,376 KB. Sidewinder Drilling Rig # 63 circulated the casing and hole while a replacement pump truck was delivered. Universal pumped 50 barrels of a Barite weighted spacer followed by five (5) barrels of water. A 50 barrel spacer of Bond Rite was followed by ten (10) barrels of water. A bottom plug was dropped and chased by 305 barrels of lead cement. A tail cement slurry ( 341 barrels ) was pumped and displaced with a top plug. At 190 barrels of the displacement circulation was lost Universal continued to pump the calculated displacement volume. Circulation was re-established, but with a reduced flow ( trickle ). The top plug was landed and bumped to 2300 psi ( 500 psi over landing pressure of 1800 psi ). After holding the pressure for five (5) minutes without any decline Universal bled off the pressure with 3.5 barrels flowing back and stopping. All floats held. A CBL/ Temperature Log was performed by Blue Dot. The temperature log indicated a cement top at approximately 2202 ~ 2220. True vertical depth of the Point Pleasant Formation was 7513 KB. , thus giving a vertical separation of approximately 5293 to 5311. After conferring with Tom Hill ( DOGRM Regional Supervisor ) R E Gas Development was allowed to proceed without any remedial measures at this time. The annular space between the 9.625 and 5.5 casings must be monitored closely to determine if any remedial measures shall be required in the future prior to stimulation of the Kiko # 3 - H well.
2109400360 2015-02-17 Drill / Deepen / Reopen No Violations I was on the Kiko # 3 -H location to inspect the drilling operations being conducted. Sidewinder Drilling Rig # 63 drilled a 17.5 borehole to 762 KB. A large water influx @ 744 KB prevented the hole being drilled on air to total depth. Sidewinder Rig #63 was removing the hammer bit and switching over to a tricone drilling bit to finish the surface section on a fluid ( fresh water ) drilling system. A walk around tour of the solids control, air packages and mud tanks did not reveal any violations during this inspection.
