0596992146 2017-09-14 Production Wells No Violations I was on site to check the status after Patterson #571 moved off site. The rig and all drilling equipment has been removed from site. I have not received indication from the operator as to when completion operations will take place. No violations were noted at the time of inspection. I performed a walk-around inspection of the pad, and did not locate any issues of sedimentation and erosion. No violations were noted at the time of inspection.
0724486246 2018-11-19 Production Wells No Violations The well was equipped with an open flow assembly. The surface/intermediate annular gauge displayed a reading of zero psi and the intermediate/production displayed 390 psi. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
1023093112 2015-10-20 Casing String Witnessed No Violations I witnessed the cementing of the conductor casing. Express Energy Services drilled a 26 diameter hole to 120. Express installed 120 of 20 OD casing. Express Energy Services was the cementing contractor. Water was pumped ahead to confirm circulation. The cement was mudscaled at 15.6 pounds per gallon (ppg). Express pumped 48.34 bbls of cement slurry at 15.6 ppg. The cement was displaced with 39.07 bbls of water. Fifteen bbls of cement returned to surface. Displacement was completed at 10:09 am.
1096962039 2017-06-29 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The #5H well is in production and consists of an open-flow assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of ten 400 bbl production tanks, four labeled as containing produced water, and six labelled as containing condensate, and one low-pressure. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

5-producing wells 10- 400 bbl production tanks, four labelled as containing produced water, and six labelled as containing condensate 5 sand separators 5 gas processing units 5 low pressure vertical separator 5 vapor recovery units 1 horizontal separator 1 compressor station 2 oil stabilization units. Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.
