0593322295 2017-01-25 Administrative Inspection No Violations I was on location to follow-up on some possible containment issues brought to my attention by Jerry Smith (DOGRM Engineering Tech) following a pad modification inspection he conducted previously. My inspection found that this well and the #s 2H, 3H, and 4H (PN#s 2719, 2720, & 2721) have been hydraulically fractured and flowed back through portable flow back equipment. The area of concern was the liner that stimulation and flow back equipment was placed on during operations and liner in the berms surrounding the pad. The liner is in poor condition and is ripped and breached in places. The liner is no longer being used for containment and I found no evidence that any oilfield substances have breached or affected the pad material. The pad surface is in very poor condition (muddy and drainage issues) and is currently being re-graded and re-stoned following completion operations. A corrugated pipe is currently being installed to help with drainage where the lease road enters the pad and is part of the approved pad modification. The pieces of liner emerging from the pad berm are not an issue. I found no violations at this time.
1529952881 2018-06-20 Production Wells No Violations I conducted a status check of the well and production pad. The pad is equipped with four wells (permit numbers 2718, 2719, 2720, 2721) and this well is equipped with an open flow assembly. Proper identification was posted at the well and lease entrance. The tank battery is surrounded by a steel dike. The enclosed tank battery and contained less than one inch of standing water. I tested the standing water for chlorides using a low-range QuanTab, which concluded in a reading of under 33 pppm. The access road was stable and drivable. I performed an inspection of the perimeter of the pad, and erosional controls appeared to be in place and functioning.

I did not locate a surface-intermediate annular pressure gauge. I contacted operator who stated they in the process of acquiring gauges from the supplier, and an annular gauge will be installed as soon as possible.
