One 3,000 gal open-top box steel tank with a spreader bar mounted on top is located near well heads (Photo # 1). All wells are also plumbed to spreader bar at box tank. Liner with berm is inadequate; it is full of ice, production fluid, and/or precipitation. Liner is also folded down to 2" high at NE corner. Brine tank is not covered. Spreader bar sprays/splashes production fluid outside of dike.

2738230 Primary Facility 2018-06-06 Compliance Evaluation No Violations Noted Compliance Evaluation Inspection to address outstanding violations against well 3H (085-24663) for use of an open-top tank to collect/store production water (PA Code 78a.57a), and spill of production water on 12/28/17 (SWMA301). Violations originally noted on inspections from 12/28/17 (ID# 2676052) and 3/15/18 (ID# 2707008). Operator has removed the open top roll-off tank that was being used to collect/store production fluid correcting the violation. Cleanup of contaminated gravel/soil cement complete. Tank and spill violations corrected.

Site restoration violations against the ESCGP permit are still open and will be addressed on a separate inspection when restoration violations are corrected.
