2402146 Primary Facility 2015-08-14 Compliance Evaluation No Violations Noted On 8/14/15, I inspected the Robert Smith site as a compliance evaluation inspection in response to the remediation report for a spill occurring on 1/31/15 and made the following observations:
2544069 Primary Facility 2016-09-14 Incident- Response to Accident or Event No Violations Noted On 9/14/16 I performed an incident response inspection at the Robert Smith Site in response to an operator reported spill (CTS ID: 322335) and made the following observations:

•On 9/6/16 EQT Corey Giles reported a spill of 100 gallons of produced fluids to the well pad. The spill was forwarded to the WQS on 9/14/16. This constitutes a violation of 25 Pa. Code § 78.57(a) and 25 Pa. Code § 78.54 for failing to control brine and other fluids resulting in discharge to the ground and failure to prevent pollution to the Waters of the Commonwealth. •The spill was cleaned up at the time of inspection with the only observed indicator of the spill location being an area of new gravel on the pad near the production units. No violation. •Well drilling was ongoing at the time of inspection. •During general pad inspection it was noted that multiple production tanks had orange streaking running down their sides from the top of the tanks. 2614157 Primary Facility 2017-05-05 Incident- Response to Accident or Event In Compliance with Policy On 5/5/17 an INCDT inspection of the R. Smith well site was conducted in response to CTS ID: 326715. The following was observed:

>> Site was undergoing fracking at time of inspection.

>>The spill in question involved roughly 10 gallons of produced water being sprayed from from an overfilled accessory tank on the pad. Affected pad materials were collected and placed in a waste box on pad for disposal. No violation, site appears to be in compliance with policy.

>>Streaking was again noted on the outside of multiple production tanks similar to streaking noted in previous inspections.

2660140 Primary Facility 2017-11-09 Routine/Complete Inspection No Violations Noted On 11/10/17 a routine complete inspection of the R. Smith well site was conducted in response to notification additional drilling on site. The following was observed:

>>A box tank was found near the impoundment with no containment. Remediation recommended, no violation.

>>A new top soil stock pile was noted on site. It was stabilized with straw at the time of inspection.

>>The soil stock pile was noted to not have any functional controls below it other than an adequate vegetated strip. Decomposed CFS should be removed, repaired, or replaced. No violation.

>>Drilling was ongoing at the site at the time of inspection.

2660152 Primary Facility 2017-11-09 Routine/Complete Inspection No Violations Noted On 11/10/17 a routine complete inspection of the R. Smith well site was conducted in response to notification additional drilling on site. The following was observed:

2691661 Primary Facility 2018-02-02 Routine/Complete Inspection No Violations Noted Well has been fracked and is producing.
2705569 Primary Facility 2018-03-13 Routine/Partial Inspection No Violations Noted On 3/14/18 a routine complete inspection of the R. Smith Well Site was conducted in response to a notification of proposed construction activity on the pad. The following was observed:

>>The site was in a high activity phase at the time of inspection drilling appeared to be completed and the site was in apparent transition to post drilling activities. No drilling rig was present on site at the time of inspection.

>>Construction of a proposed AST had not begun at the time of inspection.

>>Many water trucks were present on site in association with the attached impoundment.

2737825 Primary Facility 2018-05-16 Incident- Response to Accident or Event No Violations Noted On 5/16/18 an incident inspection of the R. Smith well site was conducted in response to a spill reported by the operator on 5/14/18. The following was observed:

>>The operator reported that roughly 35 gallons of mixed rain water and produced water was spilled to the pad when a section of containment was moved without being drained first. Fluids were spread over a large area of the pad along the area in front of the production units per discussion with Mike Cromley at the time of inspection. This does constitute a violation of 25 Pa. Code § 78.54 and 25 Pa. Code § 78.57(a) in that the operator failed to control regulated materials in a manner that prevents prevents pollution to the ground or Waters of the Commonwealth however, the violation is regarded as immediately corrected.

>>Site inspection found no residual evidence of the spill. The operator reported that all visibly impacted gravel from the spill area had been removed (roughly two clamshell dumpsters).

2747164 Primary Facility 2018-06-29 Routine/Partial Inspection No Violations Noted On 6/29/18 a routine partial inspection of the R. Smith Well Site was conducted in response to concerns about runoff raised by a nearby resident. The following was observed:

>>An unstabilized soil pile near the corner of the pad with a newly improved drain. Controls below the pile were absent though the vegetation was sufficient. No violation, remediation recommended.

>>Another only partially stabilized soil pile with no controls below the pile was located along the accessory pad access road. No controls beyond vegetation were present below the soil pile. No violation, monitoring recommended.

>>An AST was present on the accessory pad.

>>The pad was active at the time of inspection with drilling preparations ongoing. A sound wall was present around the pad perimeter.

2761859 Primary Facility 2018-08-02 Routine/Complete Inspection No Violations Noted On 8/2/18 a complaint response inspection was conducted of the R Smith well site in response to reported concerns of inappropriate drainage from the pad. WQSS John Carson accompanied. The following was observed:

>>A top soil pile on the access road to the accessory pad remained lacking vegetation and with spotty temporary stabilization. The vegetative strip below remains in good condition. Remediation recommended, no violation.

>>Emptying partially filled containment is encouraged to aid in identifying any possible leaks. No violation.

>>No evidence of excessive or unpermitted runoff from the pad was found.

>>Box in containment appears to be collecting emergency blowdown. If so it should be properly marked. No violation.
