The Operator reported "On 3/15/2019 at 4:45 PM, a contract water truck was offloading into a pump down tank using a 2” hose ran into the top hatch of the tank. As the truck neared empty, air blowing from the hose caused the discharge end of the hose to kick out of the tank. Subsequently, the hose landed outside of containment and 20 gallons of reuse water released to the graveled well pad. The impacted gravel was immediately scraped up and placed into one roll-off box. Disposal documents will be sent to you as part of the closure summary."

At the time of inspection the area impacted by the release was excavated and backfilled with rock. The impacted material was placed in a roll off box awaiting disposal.

The Operator is in compliance with the Departments spill policy.
