2486811 Primary Facility 2016-05-19 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the Talisman Energy USA, Inc. – Wilber (03-065) well pad, located in Wells Twp., Tioga Co @1015 hours on 5/19/2016. The weather was partly sunny 65 degrees. A copy of the E&S plan, well permits, and onsite inspection log was located in the mailbox onsite. The site was last inspected by company personnel on 5/17/2016. There are 4 wells on the pad surface. As of today’s inspection, all cut and fill slopes appear to be well vegetated. Silt sock was observed below what appears to be newly constructed PCSM infiltration basins. These basins have been constructed on the NE and SW corners of the well pad. Puddles were observed on the pad surface and field screened for conductivity. Puddles on the pad surface measured between 670- 800 µS/cm. Fluid was observed in the permanent containment surrounding the produced water tanks. The fluid had a field conductivity of 170 µS/cm. No violations are noted at this time. I departed the site at 1100 hours.

2784475 Primary Facility 2018-10-04 Site Restoration No Violations Noted A restoration inspection was conducted in response to a wellsite restoration report being received by the DEP on July 10, 2018. All post construction Stormwater management BMPs are installed and the site is stabilized per the requirements of chapter 102.22. The wellsite restoration report was accepted. A notice of termination for the erosion and sediment control general permit could be accepted upon submittal, based on current site conditions. There are four wells drilled that appear to be capable of production and four additional plugged wells located at this site. No violations were noted during this inspection.
