2811626 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a routine inspection of the JKLM-Woodcock Creek Well pad 125 located in Allegany Twp, Potter Co. on 12/6/2018 @1145 hours. The weather was 30 degrees, overcast, winds SW @12mph. There was ~6" of snow observed on the ground.

JKLM has submitted a major modification to the ESCGP to expand the LOD of the topsoil stockpile area. The earth moving contractor JL Watts discovered a larger volume of topsoil covering the permitted site than what was originally calculated. The proposed modification would add 1.59 acres to accommodate approximately 40,000 cubic yards of material. Details were discussed onsite with John Watts, contractor.

As of today's inspection the access road has been rough graded to the pad area. Equipment was working on grubbing the pad area, placing the topsoil in within the original LOD of the stockpile area. Wetlands appear to be flagged with orange fencing.

The last E&S inspection was conducted on 12/3/2018, with no issues noted. No v

2835762 Primary Facility 2019-02-05 Preoperation Inspection No Violations Noted Silt sock no chip berm. 1 HU.

The Department of Environmental Protection (DEP) conducted a PREOP inspection of the JKLM Woodcock Creek Well Pad 125, located in Allegany Twp, Potter Co. on 2/5/2018 @1000 hours. Present for the inspection were Jeff Long, JKLM; Jamie Spear, JKLM and John Watts, JL Watts Contractor. No activity was observed onsite.

The site consists of ~4200' of access road, a 350' x 500' well pad, two sediment traps, and a topsoil stockpile. As of today's inspection the access road and rock-lined channels were observed installed. Soil cement and cap rock has not been completed on the well pad. The out-slope of the well pad was observed stabilized with ECB matting, with TRM matting in the perimeter channels.

The perforated drains around the perimeter of the well pad were discharging into the corner PVC pipe that outlets into the sediment traps. Both sediment traps were observed holding water and discharging due to snow melt. No violations were noted at this time. JKLM plans to spud and dril

2847366 Primary Facility 2019-03-04 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was being bore hole drilled for the surface casing to be advanced into the hole. The bore hole diameter is 17 1/2" in diameter and will have a td (total depth) of 590'. The Falcon Rig # 38 and crew were pulling out of the drilled hole at the time of this inspection. The conductor pipe that is 26" in diameter and 120' in length had the hole drilled on 2-13-19 and the pipe was set on 2-14-19 and cemented on 2-15-19. No violations noted at this inspection.
2850639 Primary Facility 2019-03-08 Routine/Complete Inspection No Violations Noted At the time of this inspection this well (conductor x surface casings) was being circulated with water to establish circulation back to surface in preparation of a perforation and cement to surface job on the surface casing because of not getting circulation back to surface on the initial cement to surface attempt. The cement that was pumped into the well on 3-7-19 that did not circulate back to surface was bond logged with a Segmented Bond Tool (SBT) to determine that top of cement was at 330'. The well surface casing was perforated to allow cement to be re pimped into the casing and circulated back to surface which was successful today (3-8-19).Cementing started at approximately 11:00 am and was completed at 12:00 pm. Halliburton did the cementing. 340 sacks of cement was pumped into the well and circulation to surface was obtained. JKLM planned on waiting 12 hours before operations into the well would continue. Normal waiting time for cement to set up is 8 hours ( mandatory). No violations noted at this i
2853162 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the JKLM Woodcock Creek well pad located in Allegany Twp, Potter Co on 3/132019 @1200 hours. The weather is 39 degrees, cloudy, winds NE 6mph, 53% humidity. The Falcon rig is currently onsite drilling the top-hole portion of the 1HU well. There are 4 wells permitted on the site, but JKLM only intends to the drill the 1HU at this time.

Ron Burdick, JKLM onsite rep notified of findings. No violations are noted at this time. I departed the site @1415 hours
