The E&S Inspection Checklist indicated that SRT performed a weekly inspection on 11/17/2016 and that no attention was needed. A note on the E&S Plan stated that the wash rack was removed on 6/7/2016.
The Department noted that Appendix A of the NOT submission indicated that a Subsurface Infiltration Bed was installed on the site as the PCSM BMP. The Department's review of the E&S Plan revealed this PCSM BMP was not included in the ESCGP. The Department contacted SWN and spoke with Justin Thomas who explained that SWN believes that it was included in error. The Department found that this permit was disposed on 10/19/2011 therefore it had expired prior to the NOT receipt date of 10/26/2016. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 602psi on the production casing and 602psi on the production line. The location is being used as a staging area for the sand trucks for the frac job at the Gaylord pad. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:15p.m. |
