The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 496psi on the production casing and 382psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:35p.m.

The well was in production. The cellar was full of gravel. Unable to check for gas at the perimeter of the cellar because the gravel was frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2747704 Primary Facility 2018-06-28 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a complaint inspection. SWN notified the Department after hours and reported that a heavy rain event overnight overwhelmed their secondary containment of the existing tank battery. SWN estimated that approximately 75 to 100 barrels of water was released when the containment failed. Today's inspection found that the water released did not indicate that it was contaminated and that there was no sheen present. Photo's 2, 3, and 4 show the area that failed and was immediately repaired. As photo's 3 and 4 show, he release did not cause damage in the form of accelerated erosion. Also present during the inspection was Frank Hoehle, Department WQS and Mike Tornes, Site Supervisor. Fracking operations have been completed. Plan is to use the existing secondary containment for drill out operations which are scheduled to take place in about one week. No violations noted.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. There were five empty 500bbl water tanks on containment from a recent stimulation and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
