2535067 Primary Facility 2016-11-04 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 14:15p.m. for a new spud inspection. Minichi Contracting Group was on site to drill the holes and run the conductor pipe. I spoke with driller Eric Miller of Minichi, the rig was drilling the hole for the 1H, they were at 235' and getting ready to pull the tools out of the hole, they hit water at 183'. When the 1H is completed, they plan on moving over to the 5H to drill and set the conductor pipe. The drill cuttings are being put into a sealed role off container and hauled to Keystone Landfill. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:40p.m.

2538841 Primary Facility 2016-11-21 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 14:10p.m., met with company man Tony Anthony, Falcon Rig 23 was on site and drilling the top hole section of the well to kick off. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 2654'. Projected TD for the top hole section is 4850'. I left the site at 15:00p.m.

2574272 Primary Facility 2017-03-21 Routine/Complete Inspection No Violations Noted The Department was on site to conduct an inspection. The E&S Inspection Log Site Checklist indicated that Ryan Klemish conducted the most recent inspection on 3/17/17. Upon my arrival I spoke to the gate guard but explained that I would only be inspecting the area along the access road where a crew was working. The site is currently being hydraulically fractured.

I spoke with Ed Stone and "Smoke" (Linde) who explained that they were repairing an area where a snow plow impacted the E&S controls. No obvious issues were discovered. The site was covered in approximately 2' of snow during the inspection.

2578177 Primary Facility 2017-03-23 Follow-up Inspection No Violations Noted The Department was on site to follow up on the Department's 3/21/17 inspection. The Department only inspected the area adjacent to the access road where the E&S controls were damaged.
2585081 Primary Facility 2017-04-18 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:35a.m.

I met with company man Ed Blystone. The 1H, 3H, and 5H wells were recently stimulated CWS (Consolidated Well Service) Rig was on site drilling out the plugs and cleaning the wells out to TD. The plugs were drilled out of the 5H and cleaned out to TD. The rig was running drill pipe into the 3H and started drilling out the plugs. There were 33 stages on the 1H, 35 stages on the 3H, and 31 stages on the 5H. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:10p.m.

2598711 Primary Facility 2017-05-16 Routine/Complete Inspection No Violations Noted I initially met with Bob Lamberton, who is part of the SWN Production Company LLC Water Team around 12:15 on the Range Unit 74, Tretter Pad. Mr. Lamberton conducts inspections of secondary containment at various SWN well sites per Department regulations. I had inquired with Mr. Lamberton as to what type of site operations where in progress at the Range Unit 13 (Tonya East) Pad as my intent was to conduct an inspection at that pad, not the Tretter Pad. Mr. Lamberton stated that flowback of the wells was in progress on the Tonya East Pad. The Tretter pad is located near the Tonya East, Range Unit 13 Pad. The pads share the same entrance road off East Lake Road. I arrived at the Tonya East Pad at 12:30. There were no SWN personnel at the site during the inspection. Flowback of the Tonya East 1H, 3H and 5H wells is in progress. The Tonya East 4H well is in production. It is apparent that natural gas is being sent to sales through the gathering pipeline as the gas production units for the 1H, 3H and 5H wells are
2604221 Primary Facility 2017-06-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:50a.m.

The well was recently completed and is in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 568psi on the production casing. There were 500bbl tanks still on site from the recent stimulations of the 1H, 3H and 5H wells, the tanks were empty and on containment, the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:35a.m.

2708090 Primary Facility 2018-03-21 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2812739 Primary Facility 2018-12-10 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
