2505036 Primary Facility 2016-07-26 Site Restoration No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Swan 1122 site (ESX10-117-0265) in Farmington Twp., Tioga Co. on 7/26/2016 @ 1100 hours. The weather was 78° and sunny. An adequate site ID was observed at the entrance to the access road. The mailbox contained the onsite E&S inspection log, a copy of the E&S plan and a copy of the well permit. The site was last inspected by company personnel on 7/19/2016 “no visible issues” noted in the comments. The Notice of Termination (NOT) and Well SIte Restoration Report (WSRR) were received by the NCRO on 6/29/2016.

The site consists of a small vertical well pad ~125’ x 150’. The cut and fill slopes are stabilized with >70% vegetative cover which appears to have been recently mowed for hay. No temporary E&S controls were observed. This site meets the requirements of 25 PA Code 102.22. It is recommended the NOT be approved. The WSRR has been reviewed and processed. No violations are noted at this time. I departed the site @1130 hours.
