2835323 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Appold 493 Well Pad in Sullivan Twp, Tioga County on 2/4/2019 at 1430 hours. The weather was 60 degrees and sunny. A large amount of the pad is covered in water due to the recent snow melt. The inspection log and E&S plan were both in the mailbox by the observed act 9 sign at the entrance of the site. The last inspection conducted by company personnel was today and noted no issues.

There are currently 6 wells producing onsite, the 1H, 2H, 4H, 5H, 6H, & 7H. The PCSM BMPs were observed as functional and containments were in good shape. All fluids field measured on the pad surface and around production equipment were within normal conductivity ranges.

No violations or issues noted today, I departed the site at 1500 hours.
