2594841 Primary Facility 2017-05-17 Routine/Partial Inspection No Violations Noted EC reading w Oakton PCSTestr 35: production tank containment--2.2 mS/cm, basin--0.20 mS/cm.

A 3 man crew is on site coating some of the production line w primer. A vac truck and driver are also on site. On site ~1305-1425.

2708161 Primary Facility 2018-03-21 Routine/Partial Inspection No Violations Noted SWEPI inspection log in mailbox near site ID sign; last inspection listed on March 18, 2018. I got a 0.34 mS/cm electrical conductivity reading with an Oakton PCSTestr 35 from a puddle the pad (broke thru ice on top). Air temp 32F. JLP
2852344 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Thomas 503 well site in Rutland & Sullivan Twps, Tioga County on 3/12/2019 at 1250 hours. The weather was 30 degrees and partly cloudy. The act 9 sign was observed at the site entrance along with the site's E&S Plan which was in the mailbox. The onsite inspection log was in the mailbox by the entrance. Company personnel last inspected the site on 3/3/2019 and noted no issues.

There is currently 3 Marcellus wells onsite (2H, 4H, & 6H). Parts of the site have been recently reclaimed. The infiltration berm to the east, reclaimed portion of the pad to the north, and the reclaimed pony pad all looked good today. No erosional issues were noted. All disturbed areas have silt sock lined downgrade of them. The constructed areas all matched the site restoration plan. Containment structures were intact and functioning, no evidence of spills or releases was noted. Areas of the pad not recently disturbed are well vegetated and stabilized.
