Upon arrival on location Nick¿s Well Plugging, LLC. have been pulling 4-1/2¿ casing out of the 7-7/8¿ hole, depth at the time of inspection was 2509 ft. Plug #1 (depth 3500 ¿ 3867 ft.) has been set. Observed large disturbed area around the well that has not been restored, some production equipment has been left behind. Noted oily patches around pick up truck left on site and petroleum waste material stored in open buckets. Well doesn¿t have a monument and posted permit number. No violations are noted at time of inspection, but sent a referral to DEP WQS ¿ Melissa Herzing for follow up inspection.
