2842432 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted Jon Sassi and I arrived onsite at 1345 to perform a routine complete inspection of the well pad. Present during the inspection were Dale Bennett, Larry Gerard, Daniel Brown, and Kevin Brady (all of BKV). The weather was a balmy 50 degrees and mostly sunny with calm winds.

New produced water tanks had just been installed onsite. The new tanks are used double walled tanks purchased from Chesapeake, old tanks have felt liner. The new tanks are being relabeled to read 'produced water' on 05FEB2019 (they are currently marked 'brine').

At the time of inspection, the three wells onsite were shut in and being x-rayed, so we were advised to keep our distance due to potential radiation in the area of the wellheads. There are three new wells slated to be drilled onsite. The conductor casing for the new wells have already been installed (finished 22JAN2019).

Diaz roll-offs staged onsite. The roll offs were mostly empty at the time of inspection. Proctor and Gamble had their own roll-off dumpsters staged onsite for
