2639886 Primary Facility 2017-09-26 Routine/Complete Inspection No Violations Noted Arrived on location at: 10:30. Weather: Clear, warm, ~ 75 degs. On location to perform routine inspection of wells on pad. This pad has a total of (3) wells on pad. All wells were in production at time of inspection. All wells were producing through tubing. Tubing production pressures were being monitored by wireless sending units with LED readouts. Tubing pressures were: 1H: 1170.6 psi., 2H: LED readout not working., 3H: 1169.9. Production line pressures were being monitored by "PCS High-Low" control panels. Prod. line pressures were: 1H: 1164 psi., 2H: 1162 psi., 3H: 1165 psi. All wells (9x5) annuli vents were plumbed to surface. These vents were SI. Taylor plugs were present for pressure checks using a crystal gauge. No gauges were present to check pressures at these annuli at time of inspection. (3) GPU units were present, one for each well. The Total Flow Control panel by GPUs was password protected and inaccessible at time of inspection. No obvious concerns were noted at wells or production units. (15)
2819648 Primary Facility 2019-03-12 Site Restoration No Violations Noted I arrived onsite at 1255 to conduct a site restoration inspection of the pad. I was accompanied by Jon Sassi (EPS-DEP).

While onsite, we observed staining on an unlabeled tank near the compressor facility that is reportedly used for oil associated with the compressor. The fluid in secondary containment had a foamy oily substance floating on top. The pad surface was also stained in the immediate vicinity of the tank. Mr. Sassi called Mike Deming (SWN) to notify him of the release. Mr. Deming stated that SWN had known about the release, and that it had occurred several days earlier but was not reported due to SWN's assertion that it was under 5 gallons. Operator, the Department requests that you inspect the area and begin remediation activities immediately. Additionally, please ensure all tanks are labeled with the name of the substance they contain as well as remove any standing fluid in the tank's containment.

2836165 Primary Facility 2019-01-24 Routine/Complete Inspection No Violations Noted I arrived onsite at 1423 to perform a routine complete inspection of the pad. The weather was 36 degrees F with moderate snow beginning to accumulate, cloudy skies, and a steady moderate wind.

The lengthy access road appeared to be in good condition with no obvious areas of accelerated erosion and sedimentation. Pad was not plowed and most of the pad surface was covered in an icy slush.

I noticed that some areas of the access slope contained exposed erodible material on the steep fill slope, providing a potential source for accelerated erosion and sedimentation. Operator, please inspect the area and determine if they need to be reseeded/mulched when weather permits.

I also noted that the one (1) produced water tank located near the GPUs on the pad was not labeled. As I continued my inspection, I observed that there was another tank on the pad located next to the compressor facility that was also unlabeled. Operator, please inspect the area and determine what steps need to be taken to remedy the aforementi
