Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0417235479 2017-11-30 Production Wells No Violations The purpose of my inspection was to document repairs to well #2-1657. Lease operator AJ Strawn reported that he had replaced parts of the well head flow line that had caused fluid to leak onto the ground. At the time of my inspection I observed that the well head assembly had been repaired and the ground area cleaned up. No additional leaks were observed. No further action is necessary.
0754412503 2008-10-21 Preliminary Restoration No Violations Inspection revealed that the preliminary restoration can be approved at this time. No violations at this time.
0852737122 2017-11-21 Production Wells No Violations The purpose of my inspection was to document the status of repair of a well head leak at well #2-1657. At the time of my inspection the source of the leak had not been repaired. I called and spoke with Rick Betta while on location. Mr. Betta reported that he had spoke with the lease operator and directed him to make repairs. Mr. Betta continued that he would contact the operator again to make the repair. I later received a voice mail from lease operator AJ Strawn to report that he had replaced the leaking nipple on the well head flow line. I will schedule a follow up inspection to document the repair.
1880572909 2017-11-01 Production Wells No Violations The purpose of my inspection was to conduct a status check inspection on well #2-1657. Production was current. The access and locations were stable. Correct identification was present. The 8.625 well head was above grade. The 4.5 production casing was equipped with a plunger lift assembly. A tubing pressure gauge indicated 350 psi. Production valves were in the open position. The U-tube assembly had a small leak that was collecting on the ground. A domestic supply connection was present. The tank battery consisted of one (1) horizontal separator, one (1) 140 bbl production tank and one (1) 50 bbl brine tank. A stable earthen dike that was free of fluid was constructed around the equipment. The separator remained in good repair and contained a current 16 day chart. The production tank remained in good repair and contained correct identification. Valves were closed and plugged. The brine tank remained in good repair. The hatch lid was open. Some staining was observed on the ground at the tank base. The staining was not live or fresh. I will contact the owner to make repairs to the well head flow line. I called and spoke with Rick Betta who stated that he would have an operator out to repair the leak. I will schedule a follow-up inspection to confirm the leak was repaired.
1951983057 2007-12-26 Completion Testing No Violations Inspection of the drilling pits revealed that the pit liners are intact and the fluid levels are acceptable. Inspection also revealed that they are grading the leaseroad . No violations at this time.
