The well is in a wooded portion along the eastern banks of the Grand River. At the time of this inspection, the well is comprised of a 8.625-inch surface casing with 4.5-inch production casing with a 4.5 to 2 reducer swage and closure gate valve which is closed. The 2-inch flow-line connection is disconnected. The area surrounding the well is heavily wooded and the wellhead is covered and overgrown with grapevine and other vegetation. There are no aboveground production tanks or other appurtenances apparent. There has been no production reported since 1999 and no annual production reports filed since 2009. The well owner/operator shall submit a written plugging schedule to Inspector Stephen Tompkins at 3575 Forest Lake Drive., Suite 150, Uniontown, OH, 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |
