0920844529 2018-08-17 Production Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, fittings at the wellhead and battery were secure. The well was in active production with active chart record. Production records were current. During my inspection I noted a layer of floating oil in the secondary containment. I estimated less the one-half barrel of oil inside the containment. The owner must remove the free oil and completely remediate the interior areas of the containment and complete these activities within 30 days or be subject to additional enforcement. 0930805354 2018-01-12 Production Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice issued for the following: oil from a previous spill had impacted the interior portions of the secondary containment berm. There had been an apparent previous attempt to clean up the oil which had left behind oil-soaked sorbent pads. I had directed the owner to remove any free oil from the berm, and clean up any oil-impacted soils and structures, and remove the oil-soaked pads. At the time of this inspection, ponded water in the berm was free of oil, and the oily pad had been removed. There was no evidence that any leakage of oil was on-going. The requirements of the compliance notice were met.
1175277827 2017-06-29 Production Wells No Violations I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following: A production well with pump jack system and associated tank battery located within an active sand and gravel quarry. The well and battery are located at the front portion of the property, beyond the limits of the active operation. The fittings and connections were secure with the following exception: The was oil from a previous spill or release around the base of the storage tanks, within the limits of the battery. I had previously notified M & R Investments about this condition and the need to clean up the oil and oil-impacted soils. At the date of this inspection no action has been taken by the company. I again notified M & R Investments. I was awaiting a response at the time of the filing of this report.

1489084953 2017-11-16 Production Wells Violations Noted I inspected this well location as part of routine inspection activity. The wellhead with pump jack system and associated tank battery are located within an active sand and gravel mining operation. At the time of this inspection, I observed the following: oil from a previous release had impacted the interior of the containment. Residual oil could be seen on vegetation within the berm, as well as oil sheen on standing water, and several oil-soaked sorbent pads had been left floating in the containment. There was not an apparent active leak. The owner must begin remediation immediately, and must complete the following by 11/24/17: remove any free oil and oil-impacted water from the containment, remove any oil-impacted vegetation, and remove and properly dispose of oil-impacted pads. By statute, water with visible sheen may not be drained from the containment onto the land surface or be allowed to contact surface water. 1553609990 2019-03-26 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice # 920844529, regarding residual oil within the diked tank battery, from a previous oil release. At the time of this inspection, the residual oil within the tank battery was still present and still needs remediated. The residual oil had stained the existing vegetation on the inside of the dike and also was observed floating on the top of the water inside the diked battery. The well was in production, at the time of this inspection. I contacted M&R Investments Ohio, LLC and spoke with field superintendent, Bill Fitzmartin regarding the remediation needed inside the diked tank battery. Mr. Fitzmartin stated that he would come out and address the oil issue.
