0017818440 2019-04-23 Production Wells No Violations The purpose of my inspection is due to a Compliance Follow-up to number 1508155625 and Complaint number 5559. The well is idle and has no pressure at the well or at the production equipment. The well is on the plugging list of M&R Investments. Surveyor Tom Smith and I went in to the site and updated coordinates and the inspection. The well is equipped with a pumping unit, 4.5 inch diameter casing and 2.375 inch diameter tubing. The production manifold is open at the casing and tubing valves. There is no pressure at the gauge. There is [1]100-barrel steel tank and [1] 1100-gallon plastic tank. The chart meter shows no pressure at the chart. There is no pressure at the vertical separator. There is identification posted at the tank. The production equipment is enclosed within somewhat of an impoundment. Pictures were taken on the day of my inspection.

0284782290 2018-01-18 Production Wells No Violations The purpose of my inspection is due to a Compliance Follow-[1]100 barrel steel tank and [1]1000 gallon plastic tank. There is 400 gallon of material in the tank. The chart meter shows no pressure at the chart. There is no pressure at the vertical separator. There is identification posted at the tank. The production equipment is enclosed within somewhat of an impoundment. Pictures were taken on the day of my inspection.
0907426985 2017-07-17 Production Wells No Violations The purpose of my inspection due to a follow up on a complaint number 5559. Dale Smith and I walked into the well and looked at the unit. The well is currently shut in and setting idle. There is currently no pressure on the wellhead due to downhole mechanical issues. Dale stated that the well needs plugged and he will get company to get plug permit issued and plug the well. The complainant Kevin McCollister has sent a copy of the letter stating that the well has downhole mechanical issues and is not capable of production. There is a copy on file.

1508155625 2017-10-12 Production Wells Violations Noted The purpose of my inspection is due to a complaint follow--movement. The production manifold is open at the tubing and the casing valves. The gauge shows 0 psi on the casing valve. There is [1] 100 barrel steel tank, [1] 1000 gallon plastic tank with 400 gallon of salt water inside the tank. There is chart meter and a vertical separator with no pressure on them. On 3-20-17 I started the Complaint and the Complainant gave me a copy of the letter from the prior company to plug the said well. On 5-11-17 ODNR and M&R Investments met at the Zanesville office and they were told about the well. On 7-17-17 Dale Smith of M&R met me at the well site and agreed that well needs plugged due to down hole issues. He stated that he get with upper management to get plug permit issued. The well owner /operator shall submit a written plugging schedule to Patrick Shreve at 5880 Memory Rd. Zanesville, Ohio 43701 within 30 days per OAC 1501:9-11-05(B) and plug the well within 90 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1508176627 2017-03-20 Production Wells No Violations On the day of my inspection I met with a Complainant named Kevin McCollister. The Complaint number is 5559. The well is equipped with a Pumping unit, 4.5diameter casing, tubing head and 2.375tubing. The production manifold is open at the tubing and the casing valves. The pressure has equalized in the well on the tubing annulus. There is an electric motor on the unit. There is [1]100 barrel steel tank and 1000 gallon plastic tank with 400 gallons of brine inside. There is a vertical separator and the non working chart meter. There will need to be some work on the earthen impoundment. The identification is posted at the tank and the well. The production records are current with the Division of Oil and Gas.
1550855722 2019-02-21 Production Wells No Violations The inspection is a Compliance Notice follow--inch tubing. The tubing and casing valves are closed. The bull plugs have been placed at the well. The electric lines are sagging down because the poles are moving. The power pole from AEP (Ohio Power) with number 564D4-62 stamped on the pole to one of the two legs feeding the PN 119-28156 feeds line thru four additional poles then to unit. The poles are on the move allowing the live line to move toward the ground. I talked to the old pumper Ethan Bremmer on well and water broke in on well in 2014 after he took the well on his route per conversation on 2-21-19. The tank site consists of [1] 100-barrel steel tank, [1] 1000-gallon plastic tank and a vertical separator that has no pressure on the gauge. All the equipment is enclosed with a small impoundment and the impoundment is full of water. The new identification is posted at the steel tank. Brent Bear and I talked to Drew Adamo on 2-22-19 and well is on list to be plugged in 2019.

1977264781 2014-04-22 Production Wells No Violations On the day of my inspection I witnessed that the production equipment included a 100bbl tank, a 20bbl brine tank, a vertical separator, a drip, a 2 inch sales meter with a .125 inch orifice and a chart recorder. The well was capable of production and was equipped with 4.5 inch casing, 2 inch tubing, rods and a pumping unit powered electrically. All valves were open at the well head with production records up to date and identification present on the tank and at the well.
