0605220502 2016-08-04 Drill / Deepen / Reopen No Violations Met at the rig with Dick Marshall, Larry Nally (driller), Ken Varner (surveyor) and Jeff Redman to discuss location discrepancy of the rig location and the database coordinates of the well. The original survey stake was lost since the survey was done 2 years ago. Per the surveyor, Ken Varner, the plat is correct and the rig is set up at the correct location. Mr. Varner stated he put the wrong coordinates on the plat but everything else is correct. The GPS coordinates from the permit are approximately 110 feet southwest of the rig coordinates (entered). Mr. Varner measured the actual distance at 108 feet. Marshall is seeking to drill the well on his current permit even though it has expired. The well was not spudded prior to expiration of the permit. The operator did not contact the surveyor until they rigged up and the inspector shut them down for being on the wrong location. The rig is set up, the pit is constructed and lined, and a cellar has been dug under the rig floor. There is a small amount of water in the pit but no cuttings are present. Photos were taken. The operator needs to renew his permit to drill and correct his plat with the Division prior to drilling the well, per Bob Worstall and the permitting staff. Mr. Marshall wishes to have a written statement that he is not permitted to drill the well given to him and said he would sue the farmer and the Division. He is also considering his avenues for appealing the decision of the Division. He requested permission from the inspector to drill and was denied. He requested the inspector to stay away from the rig until the well is drilled which was also denied.
0744407512 2019-05-16 Drill / Deepen / Reopen No Violations Visited location on 05/16/2019 to check on the status of drilling operations. Jim Nally Drillings cable tool rig was drilling ahead at 300. Expect to cement surface casing Monday morning. No problems were noted at the time of the inspection.

The driller previously marked a location in the field that corresponds to the permit coordinates. The operator was advised he will have to buy a new permit and resurvey his location. The surveyor was supposed to be there this week but has not resurveyed the well to date.

The survey stake is no longer present. Marshall was advised to resurvey the well or move the rig to the appropriate spot. The surveyor wont be available until next week and the permit will be expired. Marshall doesnt want to buy a new permit if the surveyor shows him to be on the correct location. He was advised that if the well isnt started by the expiration date he will need to.

1680986631 2019-01-11 Drill / Deepen / Reopen No Violations Arrived on location on 01/11/2019 to check on the location prior to beginning drilling operations. Jim Nally Drilling Rig #1 was rigged-up onsite. The pit has been dug and lined, but a new liner will be required, as this one is old and has several holes. Plan to spud the well Saturday.

Nally marked a spot in the field corresponding to the well coordinates in the database. The original survey stake is no longer present. Larry asked if they would need to get a new survey done before the well was drilled. He also stated that the owner, Dick Marshall, believes the well is supposed to be located approximately 110 feet to the northeast of the location marked by Nally. Nally was advised that they need to drill on the location matching the coordinates indicated in the database or they will have to have the location re-surveyed.
