0767274035 | 2015-07-21 | Production Wells | Violations Noted | On the day of my inspection I found that the wellhead was equipped with a pumping unit. The motor was intact, the valves were open and the polished rod showed evidence of recent use. The tank battery consisted of two 100 barrel tanks. I found a three foot area of paraffin around the wellhead. I found that the flow line from the well to the tanks had leaked, leaving a 23 x 5 swath of bare muddy soil and a 2 circle of oil near the leak. The mud was heavily tracked by deer. I tested standing water in a deer track with a Quantab test strip. The test came back positive for chlorides at 423 parts per million (ppm). I also found that a line leading to the tanks from the direction of CR 18 had leaked, leaving a 10 x 2 area of crude oil contamination. The tank battery is shared with permits 4868 and 4869. The wellhead was not identified and the permit number at the tanks was wrong. There have been no production reports submitted for this well since 2011, and the previous five years showed 0 bbl. Oil and 0 mcf gas. Remove all crude oil and brine contamination, post identification at the wellhead, correct the permit number at the tanks and submit annual production reports for 2012, 2013, and 2014 within 30 days. |
1166688598 | 2015-08-17 | Production Wells | No Violations | On the day of my inspection I found that the clean up had been partially completed. There was still contaminated soil where the lead line had leaked, The identification was still not in a conspicuous place and the production reports had not been filed. I met with landowner Alan Kukla at the well. He claimed that he was assisting Clayton King with the clean up. I pointed out the areas that still needed attention. |
1441202397 | 2015-09-02 | Production Wells | No Violations | On the day of my inspection I found that the areas of contamination had been cleaned up. The identification had been corrected and posted in a conspicuous place. The production reports were still not up to date. I called Dale McDonald, he claimed that Clayton King has payed him for the well and he was taking care of the reports. I called Clayton King. He claimed that he would get the past production reports submitted for Dale McDonald. He also agreed that he had purchased the well and he was waiting to apply for a transfer because the well has an active violation. |
1507814256 | 2017-10-11 | Production Wells | No Violations | On the day of my inspection I found that the well was equipped with an electric powered pumping unit. The motor, drive belts and electric were intact. The flow lines were connected and the valves were open. The production equipment consisted of two 100 barrel steel tanks in a shallow earthen dike. Correct, legible identification was posted at the tanks and there were no leaks or spills. |

2085256036 | 2016-01-21 | Administrative Inspection | No Violations | I spoke with Gregg McDonald about getting the production reports brought up to date for this well. He claimed that his wife has been submitting reports. After some conversation I discovered that they have been reporting for permit 4663, a well that was plugged years ago. I advised Mrs. McDonald to submit production reports for 2012 through 2015 under the correct permit number. |
