Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0155478500 2017-10-18 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well pad name is Myron and it consists of 5 wells. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Annular pressure monitoring was present. The gauge on the production casing read 330 pounds per square inch and the gauge on the production tubing read 230 pounds per square inch. The production equipment consisted of eight 400 barrel tanks within steel line containment and five gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and two combustors. A dust suppressant had been recently applied to the lease road. The identification was located at the wellhead and the production records were current.
0252259442 2013-10-29 Drill / Deepen / Reopen No Violations My inspection today finds Patterson Drilling is drilling at a depth of 11108. The containment appears to be functioning properly.
0254406293 2013-10-17 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig is at a depth of 3863 drilling a 12.25 hole.
0304669641 2015-07-29 Production Wells No Violations My inspection today finds the well properly equipped to be capable of production. While on location I witnessed one Myers truck loading brine and one Gibbons truck loading product at the tank battery. The tank battery consists of eight 400 barrel tanks set in a liquid tight steel containment. The tank battery is shared by four other wells on the pad.
0372529033 2016-07-28 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery consists of eight 400 barrel tanks set in a plastic lined liquid tight steel containment. The tank battery is shared by four other wells on the pad. Heavy rain is falling at the time of inspection.
0639794995 2013-09-04 Casing String Witnessed No Violations My inspection today finds Falcon drilling ran 18.625 casing to a depth 365. Allied cemented the casing in place with 733 sks of class A cement.
0707735855 2018-11-26 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.375-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 140 pounds per square inch, the 9.625-inch diameter casing had 0 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The Gas Processing Units and midstream valve site were on a secondary pad to the southeast of the well pad. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
0728032801 2014-06-12 Preliminary Restoration No Violations My inspection today finds the well site meets the preliminary restoration requirements under 1509.072(A) of the Ohio Revised Code. The tank battery is comprised of eight 400 barrel tanks set in a liquid tight containment system.
0974876465 2017-08-02 Production Wells No Violations The pad name is Myron and it consists of 5 wells. The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of five gas production units, eight 400 barrel tanks, two compressors, and two combustors. Identification was present and production records were current. Deep Well Services were on location completing a material modification alongside of the lease road.
1039447485 2016-02-25 Production Wells No Violations My inspection today finds the well properly equipped to be capable of production. The tank battery consists of eight 400 barrel tanks set in a plastic lined liquid tight steel containment. The tank battery is shared by four other wells on the pad. The rainwater drainage sumps are open at the time of inspection.
1133879158 2013-08-06 Casing String Witnessed No Violations Express Energy Services on site with conductor rig they drilled to 100 with 32 bit and ran 100 of 26 casing and Allied Cementing pumped 336 sacks of Class A cement with a weight of 15.6 and a yield of 1.18 and they had 10 bbl of good cement returns to surface.

Note: Quantab reading taken on mix water indicated that it was fresh water.

1379416727 2013-09-08 Casing String Witnessed No Violations Falcon rig #20 ran 1987 of 13.375 casing. Allied cemented the casing in place with 2170 sks of class A cement. I witnessed 120 bbl of cement returns.
1512395990 2017-12-01 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well pad name is Myron and it consists of 5 wells. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Pressure monitoring was present on all casing strings. The production equipment consisted of eight 400 barrel tanks within steel line containment and five gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and two combustors. The identification was located at the wellhead and the production records were current.
