The pad consists of 8 producing wells. The tank battery sits on the left side of the pad as you enter it and consists of (8) 400 barrel tanks. Identification is present on each well head. The tank battery consists of eight 400 barrel tanks set inside of a plastic lined steel containment system. On the opposite side of the pad there are eight production units, also set in containment. Several pieces of construction equipment are parked on the location and are being used for the reclamation of the adjacent fresh water impoundment.
0987147452 2013-12-29 Casing String Witnessed No Violations My inspection today finds the drilling rig, Falcon 20, ran 18.625 casing to a depth of 415. Allied cemented the casing in place with 838 sks of class A cement. They pressured up to 620 psi after landing the plug. The casing string held with no pressure loss.
1018359782 2015-12-16 Production Wells No Violations My inspection today finds the well has been placed into production. All work on the South side of the out slope has been finished. Three temporary tanks have been placed on the location, in containment, to allow the wells to be produced while a small slip is repaired behind the permeant tank battery. All temporary equipment meets the spacing requirements of section 1501 9-9-05 of the OAC.

1559826728 2019-01-31 Production Wells No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place on the 9.625-inch diameter deep intermediate casing, and 13.375-inch diameter intermediate casing. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid storage tanks, and a combustor that was shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a midstream valve site on the west side of the pad. As well as a water impoundment to the south, right side of the lease road as you enter the pad. Identification tags were present, and production was up to date.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of eight gas production units, eight 400 barrel tanks, and two combustors. Identification was present and production records were current. Antero Resources was on location preforming maintenance on a gas production unit.

The pipeline tie in was located on the West side of the pad. The identification was located at the wellhead and the production records were current.
