0746389309 2019-05-07 Plug / Plug Back No Violations I arrived to the rig at 7:05 AM. The wood plug was removed from the 8 inch casing. Plants & Goodwin installed a wellhead to the 8 inch casing. I observed 2 two inch hoses connected to the wellhead that was placed into a half-round tank. A Mullins Energy water truck UIC#25538 delivered to loads of fresh water to the rig location. At this point Plants & Goodwin had wellhead control measures in place. After the wellhead was installed on the well Tim Krzos ran an impression block on tools and tagged an obstruction at 440 feet. The impression block did not reveal the obstruction. Tim Krzos then ran a 7.25 inch bit in the well and began drilling. It appeared that the obstruction was a cement plug as samples from the well being baled revealed a dark gray substance. Drilling continued until 460 feet. At that depth the drill bit started to stick in the well. Tim Krzos said he believed the bit was at a casing seat point and the reduced hole was causing the bit to stick. According to records from API number 34-089-6-1082-0000 the hole size would have been reduced at approximately 460 feet. It was then decided to stop drilling and install 5.5 inch casing to 450 feet.

1438712616 2014-05-21 Production Wells No Violations Inspect well in regards to phone call from landowner Mr. Jason Blowers. Inspection revealed a leaking well on the property. The well was leaking brine and gas. A large area of stressed vegetation was observed as far as 75 feet away from the well. A Quantab test strip was placed in the brine near the well and resulted in an 8.0 on the high range scale which equals >6000 ppm for chlorides. Casing was found below grade with a wood plug in it. Orphan well inspector Jeff Fry will be contacted regarding this well.

There is a new wooden stake with a plastic marker ribbon at the well that appears to mark the location as a survey stake. The well continues to intermittently leak natural gas and fluids as before.

Granville Township Fire Department Lt. Ryan Hussey was also present for the safety meeting. The rig was prepared to clean out well. Time: 1:00 PM.
1839287378 2019-05-01 Plug / Plug Back No Violations My inspection revealed that a Bucyrus Erie 28 cable tool rig has been delivered to the well location. I also observed a tool shed and half tank on location. No personnel were present. Time of inspection: 3:00 PM.
2077668022 2019-05-08 Orphan No Violations I am on location today to witness and document the activities performed to plug this well. Today Plants and Goodwin Inc arrived on location and waited on the 5.5 inch diameter casing to be delivered. Once the 5.5 inch diameter casing arrived the rig crew unloaded the casing onto there float so the could tally the pipe and run it in the hole. The rig crew unloaded 17 joints / 526 feet of casing but will only be running 450 feet total. They had to cut one of the joints so they could get to the 450 feet in the hole. The company brought there cement truck down today and put it on location so it would be ready for tomorrow.
2109963785 2014-08-06 Orphan No Violations Inspect well as status check. Working with landowner to place well into orphan program. Well appears to have 8 casing with wood plug shoved in the top. Dug around the 8 casing approximately 1.5 feet below grade and could not find any other casings.
