0100573243 2006-10-18 Production Wells Violations Noted A complaint was received alleging that the Parley and Lowell Nutter wells are idle. According to the owner of Chieftain Energy, Mr. Doug Kitchen, and the landowner, Mr. Lowell Nutter all seven wells are tied into each other to produce house gas. Mr. Nutter states that little gas is obtained from these wells. Mr. Kitchen admitted that a few wells need some work. Mr. Kitchen will be given credit for 1 well since some house gas is generated. The other 6 wells must be plugged or produced. Mr. Kitchen was given a verbal NOV on 10/18/2006.
0537491107 2009-03-17 Production Wells Violations Noted A complaint was received alleging that a flowline has leaked oil onto the ground surface, in a pasture field, located on Maysville Williams Road. An inspection of the flowline revealed that a 2 steel line from this well has leaked approximately 2 (length) x 5 (width) of oil in pasture field near a creek. At this location, the flowline contained a metal band clamp that appears to have been used to repair a previous leak. The oil does not appear to be fresh and the well is shut down. Note: Class B Wastewater from the City of Logan is applied to the pasture field. The well owner has been contacted by telephone and instructed to perform repair and cleanup.
0568200906 2009-04-06 Production Wells No Violations A follow-up inspection revealed that the flowline has been repaired. A new band clamp has been placed around the steel casing. Chieftain Energy has been informed that if line locations leak again, the lines would have to be replaced.

An inspection revealed that the well was shut-in and contains a shared tank battery. The top of well head contains gas leak and will need to be addressed. RBDMS shows no statement of production filed since 2006. Chieftain Energy Corps. attention shall be directed to Ohio Revised Code Chapter 1509.062 (A)1, which states that for an existing well that has no reported production for two consecutive reporting periods, as reported in accordance with section 1509.11 of the Ohio Revised Code shall plug the well in accordance with section 1509.12 of the Revised Code, obtain temporary inactive well status for the well, or perform another activity approved by Chief of DMRM. From years 2003-2006, the reported production for this well showed 9, 7, 11, and 12 bbl crude oil with no gas production. The reported production is below the required 15 bbl requirement under section 1509.062 (A)2. No reported production since 2006. Failure to comply will result in further enforcement action.
1295020632 2010-12-03 Production Wells No Violations A follow-up inpsection revealed non-compliance. The valve for the pumping tee has now been opened and casing valve remains closed. At time of inspection, statements of production have yet to be submitted to the Division. Currently, there are no natural gas sales from this lease. Note: The landowner, Mr. Lowell Nutter, alleges that well has not been pumped since, the last time they were sent an NOV (2006). Mr. Nutter requests plugging and abandonment of well, if company will not produce the well. Note: The flowline from this well consists of approx 1.5 steel line with half partially routed above ground alongside a creek before disappearing underground to shared tank battery. Chieftain Energy unsure of location underground before tie-in to battery.
1339441971 2010-10-04 Production Wells No Violations FOLLOW UP TO NOV # 1288292020 The Division received a landowner complaint alleging non-production from this well. 1447850905 2006-12-26 Production Wells No Violations A follow-up inspection revealed that the previously noted violation has been resolved. Mr. Doug Kitchen and Mr. Ron McBride stated that they replaced the piston rings and performed work on the cylinders. The well was observed pumping for 15 minutes. The company stated that the well exhibited no problems and has been running for 45 minutes.

2071214106 2006-12-22 Production Wells No Violations A follow-up inspection was conducted on 12/22/2006. During the inspection, it was noted that service work would need to be performed on this well. The company was unable to keep the well running, due to a compression problem. Mr. Kitchen was instructed to have this well running the morning of 12/26/2006, or this case would be handed over to management.
