0516851509 2012-09-17 Orphan No Violations A follow-up inspection revealed that the well consisted of 4.5 casing equipped with wellhead. An additional casing (drive pipe) was also buried. Dug around and observed approx 8 casing. The well contained rods and tubing with no pumping unit. A slight gas odor was observed from the well. Oilfield debris exists around the immediate area. The well is situated on a property designated as agricultural field with no overhead obstructions. An access/lease road will need to be constructed and may require a culvert at the entrance. Note: Well location(s) were previously scaled and have now been corrected with GPS readings.
0938936554 2018-07-03 Final Restoration No Violations Final restoration is not approved. Plugging contractors equipment has been removed from site and wells have been cut off and backfilled. Plugging contractor will need to grade driveway used for access before final restoration will be approved.

1532694642 2018-05-15 Plug / Plug Back No Violations Witnessed the installation of 50 sacks of Class A Cement for the purpose of the bottom plug of this well. The tubing was set at a depth of 1246 feet and the tubing size was 1.5 inch.
1914391746 2012-12-06 Orphan No Violations A follow-up inspection conducted with Orphan Well Program Supervisor, Mr. Gene Chini revealed a slight change to the well status since the last inspection, as no gas odor was smelled during the inspection. The well consisted of 8 casing (buried), 4.5 casing equipped with a wellhead, rods and tubing. The well is situated on a property designated as agricultural field. During the inspection of all wells located on this property, the landowner, Mr. Donald Schmitz confirmed the installation of field tile. Mr. Schmitz stated that the tiles were placed 20 apart and routed to the north and south. Steel tanks will be utilized in place of earthen pits. Mr. Schmitz requested for casing to be cut and buried 4 below ground level. A lease road does not exist for wells located on this property. However, the best access to the wells would be a gravel entrance to the field located off N. Star Fort Loramie Rd and not Lightsville Northern Rd.
1946648881 2018-05-21 Plug / Plug Back No Violations Ripped the 4.5 inch production casing at a depth of approximately 500 feet. Pulled 6 inch surface casing at the same time that production casing was pulled.
