1312815617 2011-08-05 Production Wells Violations Noted An inspection of the well and tank battery revealed no ID. It appears a tank had been leaking brine and has been replaced, there is contaminated materials around tanks and approx. 50ft away a tree has died in a yard. Done a field test around tree with Quantabs and got a reading over 2000 ppm of chlorides. No kill area noted and grass appears to be ok. No production report submitted since 2008 and dike does not meet SPCC requirements. Need full and legible ID at the well and tank battery. Clean up and remove any and all contaminated materials around tank battery and in yard. Build a dike to meet SPCC requirements. Submit production records and records of brine being hauled. Seed and mulch areas as needed. Submit production records to Columbus Office and Brine records and disposal slips for contaminated material to Inspector. Failure to comply may result in legal actions.
