The wellsite currently has 2 christmas tree wellheads onsite and no production equipment. New erosion controls have been in put in place and vegetation has not been established yet on one hillside. I will check back in a month or two when the vegetation has had a chance to grow, allowing preliminary restoration to be passed. There are no site violations at this time.

1294716965 2013-10-29 Preliminary Restoration No Violations I was on location to pass preliminary restoration on this well. At this time all of the drilling equipment has been removed off location and there is currently only a wellhead on location at this time. Preliminary restoration can be passed at this time.
1418441654 2015-03-18 Production Wells No Violations On the date of inspection I am on site with SE Regional Manager Brent Bear to inspect the well for placement into Temporary Inactive Status due to a lack of pipeline infrastructure. The well is equipped with a Christmas tree and is currently shut in and chained. At the time of my inspection there is no production equipment on site. There are two wells located at this well pad and there is no identification. The cellar is full of fluid and will be required to be pumped out. I contacted the Chesapeake field office and spoke with Manny Johnson. Mr. Johnson informed me that he would have the local well tender come to the site and address the issues as soon as possible. After Chesapeake has corrected the above issues, the well can pass field inspection for Temporary Inactive Status.

Surface and intermediate annular pressures read 0 psi. Production equipment includes two sand seperators, two GPUs, and two 400 barrel steel tank, within their own steel containment. 1585265199 2018-09-18 Production Wells No Violations On the day of my inspection I checked production at the request of Landowner candy Dailey. Landowner does not believe they are receiving royalties for production. I provided her with Landowner relations number in Oklahoma. Chris Huey, production foreman, gave me the number. This plunger lift well was in production during my inspection. Surface annular pressure was zero and intermediate annular pressure was 60 psi. Tubing pressure was 866 and production casing was 930 psi. Production equipment includes two 400 barrel steel tanks, two sand seperators, and two GPUs. Production records are current. Landowner phone number is 740-543-4317.

1928654116 2012-10-24 Production Wells No Violations My inspection found that the lease road was well maintained with a locked gate. The well pad was fenced and contained two wells with assembled christmas tree wellheads onsite. The well contained a large amount of rainwater but it was contained on the pad and there was no water leaving the pad. The wells were surrounding by steel fencing and cement barriers. There are no violations at this time.
