1496666296 2017-06-05 Casing String Witnessed No Violations On site inspection to witness the mine string surface combination cement job with Pioneer Drilling rig 71 and O-Tex Cement Service along with Inspector Andrew Corder. The rig has drilled a 24 diameter surface borehole to a measured depth of 610 off of a drilling rig Kelly Bushing height of 28.6. The rig installed 608 of 18.625 diameter 87.5 pound per foot J-55 grade buttress thread casing with a casing guide shoe and float collar ran and a World Oil Tool swell packer set at 461, 4 casing centralizers and 2 cement baskets were also installed in the string. O-Tex cement service cemented the casing in 2 stages with the bottom section cemented with 43.5 barrel cement mix consisting of 202 sacks of class H cement with the cement weight verified at 15.6 pound per gallon by weight of scale and the cement displaced to the float collar with 173 barrels of fresh water, upon the landing of the plug at 170 psi pressure was increased in increments up to 560 psi and the casing was pressure tested at that point. The swell packer element was designed to set at 450 psi. The rig then proceeded to run 1 diameter tubing down the back side of the 18.625 casing to a depth of 438, the annular space was then topped off with 151 barrels of class H cement with 25 barrels of cement returns witnessed to surface.
1558446188 2017-06-07 Casing String Witnessed No Violations During this inspection Pioneer Rig 71 had completed running the 13.375 inch diameter casing string and anticipate to cement the same when cementers show up on location. I spoke with Inspector Bill Stolarik and he informed me that Inspector Austin Gutridge would be covering this cementing job. See his report for execution of this cement job.
1823367444 2017-06-05 Casing String Witnessed No Violations Onsite inspection conducted accompanied by Inspector Bill Stolarik. Pioneer Rig 71 had reached the total depth of the 24 inch diameter open hole section of this well bore at a depth of 608 feet. The 18.625 inch diameter casing was ran to total depth. O-Tex Cementing Services ran 43.5 barrels of Class H cement to stabilize the bottom of the casing. The inflatable packer element was then expanded by pressure to allow the top off job to ensue. O-Tex then pumped 151 barrels of Class H cement through 1 inch diameter tubing to established cement circulation to surface.
2099297909 2018-11-14 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. All erosion control filter sock material placed on the outward slopes during construction have been removed. The well pad and lease road are stone construction and in good repair. Signage is posted at the lease road entry point and at each well head with hazard warning labels on the tank battery. Emergency shut down devices and fire extinguishers are situated at each critical area of the well pad. There is a 3rd party mid stream operation along the lease road on the left side entering the well pad. The lease road does have a gate to restrict access. A total of 6 wells are present on the pad with the said well equipped with an A&B section well head along with a tubing head production tree. 2 diameter riser pipes with valves and gauges are present and attached to the well head A&B sections for the purpose of annular pressure monitoring. The A section gauge which is monitoring the 13.375 casing pressure reads 600 pounds per square inch pressure (psi) the B section gauge which monitors the 9.625 diameter casing pressure reads 1,100 pounds pressure. The 5.5 diameter casing pressure is 2,000 pounds pressure and the tubing head pressure before the choke valve is 1,000 pounds pressure. The production tubing well head valves are open to the sales line and an emergency shut down device is situated on the production well head tree. There are (6) gas processing units present on location and on the rear left hand corner of the well pad is the production fluid facility that consists of (4) steel construction 400 barrel fluid volume tanks along with a 50 barrel steel tank. All situated inside a polymer lined steel containment dike area.
