Karen stated that she is currently out of town but would let someone know the status when she returns to the office on Friday. When asked about a return call timeframe, Karen replied by saying give me until next week.

1228605621	2016-06-08	Plug / Plug Back	No Violations	I contacted Mr. Brown at 330-436-1395 and advised him that we have not received the report for the plugging of this well from November of 2013. I left a message to inform Mr. Brown that he is responsible for filling out the plugging form when a Inspector is not on site to witness it. Next I contacted Karen Reese and ask if she could fill out the forms and provide the Division with the plugging affidavit. She advised me that Mr. Brown was the person who had all of the information and would be the one to fill out the form. I advised that he had not returned any prior calls from the Division. She gave me number of Greg Passard who is his Supervisor. I contacted him at 330-315-6967 and left a message. I also called his cell phone at 330-221-2021 and left a message about the forms and no response from Mr. Brown.

The tubing is flowing oil currently due to the well gassing but it is being contained. The first cement plug will be set tomorrow.

Karen was instructed to complete and submit to the Division a plugging report that is signed by the owner, notarized, and contains a copy of the cementing tickets. Karen stated that the failure to submit the plugging report was a miscommunication, but would make sure this report is completed.

1628977053	2018-03-26	Final Restoration	No Violations	On the day of my inspection I was unable to locate a well at this location.Previous inspection reports indicate it was plugged in 2013. I used GPS point and google earth aerial photo dated 5/2012 to verify previous location of well. Area has been regraded and is completely vegetated. Form 55 was located in Columbus RBDMS. Site meets requirements of 1509.072B for final restoration.
