1343224509 2012-07-25 Production Wells No Violations The entry way to the lease road from St. Rt. 83 going towards Cumberland Oh, the land owner Curtis Stump has recently installed a gate with a lock to keep the well owner Mike Montgomery from having access to his well, Mr. Montgomery has meet with me and gave a copy of the documents allowing him to use this access. I am meeting with Mr. Montgomery on 07/26/2012 at 7:30 A.M. to approach Mr. Stump with the documents to unlock the gate.
1512397651 2017-12-04 Production Wells No Violations On the day of my inspection I met with A.R. Crawford owner OPM777 owner A.R. Crawford owner to discuss Compliance Notice-1786313805. We went to alternative access road which was overgrown with heavy vegetation and downed trees. Arrival at the well was equipped with 8.625 inch diameter casing; 4.5 inch diameter casing and a casing lift system with all valves in the closed position. The production equipment included a disconnected 210 barrel tank, a 100 barrel tank within an earthen dike, a horizontal separator, a drip and a 2 inch meter run with a .375 inch orifice and a chart recorder with no chart. No identification was present and production records were current for 2016 with zero production reported since 2012. The disconnected 210 barrel tank has an area of 4 foot in diameter near the burner flange which is contaminated with oil. Mr. Crawford and I met with land owner Mr. Curtis Stump to discuss access to the well. Mr. Crawford will revisit the well this weekend and draft a plan to remove the tank and get the well back online.
1533728790 2018-08-08 Production Wells No Violations The purpose of my inspection was to follow up on compliance notice -1786313805. The compliance notice stated the following; well operation causing pollution and contamination, failure to legibly identify well, and well shall be plugged. During my inspection all the original violations still existed. The 210-barrel tank had a small area of oil contamination that was thin and decreasing in size around the burner tube, the well was not identified, and the well was not plugged. The well was equipped with 4.5-inch diameter casing with a ConCoyle system with production valves in the closed position. I called the well owner, AR Crawford, at 8:05 a.m. on 8/8/18. Mr. Crawford stated that there was lease issues that kept him from accessing the well, but those issues are now resolved, and he has just received permission to re-enter the well through the original lease road. Mr. Crawford plans to remove the 210-barrel tank, clean up any oil contamination present, and get the well back into production. Mr. Crawford stated that he would call me once he has completed the necessary actions to gain compliance.
1554904921 2019-04-10 Production Wells No Violations The purpose of my inspection was to follow up on compliance notice -1786313805. The well was equipped with a Concoyle casing lift system with production valves in the closed position. The production equipment consisted of one 210-barrel tank, one 100-barrel tank, and a horizontal separator. The 210-barrel tank that caused the pollution issues has not been removed and the contamination has not been remediated. The lease road was impassable by vehicle due to multiple down trees.
1786313805 2017-11-20 Production Wells Violations Noted On the day of my inspection I met with land owner Curtis Stump who stated that the correct access road comes off of Cooper Road which is overgrown. I gained access to the well and found it was equipped with 8.625 inch diameter casing; 4.5 inch diameter casing and a casing lift system with all valves in the closed position. The production equipment included a disconnected 210 barrel tank, a 100 barrel tank within an earthen dike, a horizontal separator, a drip and a 2 inch meter run with a .375 inch orifice and a chart recorder with no chart. No identification was present and production records were current for 2016 with zero production reported since 2012. The disconnected 210 barrel tank has an area of 4 foot in diameter near the burner flange which is contaminated with oil. The well owner/operator shall remove the 210 barrel tank, remove contaminated soils and remediate the affected area. The well owner/operator shall submit a written plugging schedule to Doug Bennett at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1923472990 2012-07-26 Production Wells No Violations Mr. Micheal Montgomery contacted me to inform me that Mr. Curtis Stump would not allow him to have access to his well to remove a production tank that has a leak in it, Mr Montgomery called for a sherriff to assist him in getting access to his well, the sherriff and Mike Montgomery went to Mr. Stumps but was unable to get access with out going through the court.
