0180155295 | 2013-09-22 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Precision Rathole has set a 30 inch sleeve to a depth of 12 feet below the base of the tin horn. They drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch conductor casing (94 pound per foot with an inside diameter of 19.124 inches). O-TEX cemented the casing with 250 sacks of ClassH cement 2% CaCl, 1/4 pound per sack cello-flake, and 0.4% C-41P. The cement had a density of 15.6 pounds per gallon, yield of 1.20 cubic feet per sack, and they used 5.25 gallons of mix water per sack. O-TEX pumped 53.43 barrels of cement slurry, pumped 39 barrels of displacement, and there was 10 barrels of cement returns to surface. My inspection revealed no violations at this time. |
1272224210 | 2015-10-23 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

1389667089 | 2013-12-31 | Drill / Deepen / Reopen | No Violations | I was on site to check the status of Nomac Drilling Rig # 77. My inspection revealed that Nomac Rig # 77 was drilling the lateral section of the well at 12,100 feet. My inspection revealed no violations at this time. |
1889111933 | 2017-03-07 | Production Wells | No Violations | I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #10H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.
The equipment on the pad consists of the following: 4 producing wells 1 Low pressure vertical separator located inside of dike 4 sand separators 4 gas processing units 1 vapor recovery unit 1 heat treater 1 compressor station There was approximately four inches of standing water inside of the dike. I tested the standing water with a low-range QuanTab which concluded in a reading of under 30 ppm. |
