2797557 Primary Facility 2018-10-29 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Devonian Resources Inc. conventional well (033-26466) on October 29, 2018. The area surrounding the production equipment has been restored. Restored site is well vegetated. No signs of E&S issues. The chart recorder is labeled with the operators name, operators contact information, and well permit number. No date was observed on the chart recorder. Flakey paint and corrosion were observed on the well head, separator, and axillary piping. The Department recommends monitoring the production equipment and replacing and/or repainting the equipment if necessary.

Department staff used an Altair 5x gas meter during the inspection. Combustible gases were detected at 15% LEL on some of the axillary pipes connected to the well head (see photo). The Department recommends addressing the combustible gas issue.

This case will be referred to a Department Oil & Gas Inspector for further investigation.

2804933 Primary Facility 2018-11-13 Follow-up Inspection No Violations Noted The weather is 40 Deg. F and cloudy at time of inspection. Follow up inspection of Devonian Resources Inc., Shaffer Nancy Pad Well # 2 on November 13, 2018.

There is 1 producing well on this pad. Production processing equipment includes separator and production water tank, methanol bottle; measurement equipment - orifice meter station. Additional meter has been installed for the residential consumer. Annulus (7 x 4-1/2) was closed upon arrival to location. Company operator opened annulus during inspection, observed foam coming out of the vent. Left vent closed. Detected gas leak (55%LEL, 3% combustible gas) at the meter line. Verified pressures: casing pressure - 450 PSI, line pressure ¿ 90 PSI. No violations are noted at time of inspection, but discussed further repairs and follow up inspection with company owner that is present during this inspection. Devonian Resources Inc.
