2364125 Primary Facility 2015-03-12 Routine/Complete Inspection No Violations Noted A routine Inspection performed in conjunction with a water supply complaint inspection (ID#2364119). Proper signage and all records including the ESCGP, well permits, PPC, ERP, containment plan, E&S plan and drawings and PCSM plan located on-site. There is a freshwater impoundment located on-site which had little water stored during this inspection. There is currently no drilling activity on-site. There is one existing well (125-23192) and three wells (125-27425, 125-27435 and 125-27436) have been spud and vertical portions drilled between 12/23/14 and 1/3/15; the laterals plan to be drilled in the spring/summer. 5 additional wells were recently permitted but have not been spud as of this inspection. The diversion channels (channels 1 &2) along the backside of the FW Impoundment (E) may need re-graded as there was some water ponding. All areas on-site appear to be temporarily stabilized with matting and all of the E&S controls are installed and appear to be functioning appropriately. There were no violations noted during this inspection.

TD was at 10,796 ft. MD (7691 ft. TVD) on 1/9/2016. The 5 ½” casing was set at 10,792 ft. The estimated top of cement is at 3501 ft.

2593312 Primary Facility 2017-05-15 Routine/Complete Inspection No Violations Noted On Monday, May 5, 2017, I completed a routine-complete inspection at the Cashdollar Well Site located in Amwell Township, Washington County.

The Act 9 sign was in place and clearly legible at the site entrance which is secured with a gate and a lock. The access road and well pad are in good condition and the site is stabilized with vegetative growth. The operator may consider re-evaluating the vegetation on-site to determine the necessity of keeping the filter sock and silt fence in place or whether it can be removed. The operator should also continue their periodic monitoring program and address any deficiencies, signs of accelerated erosion and/or sedimentation upon identification.

The secondary containment structure around the production tanks appear to be in good condition and functioning as intended. The gas wells appear to be in good working order and are labeled as required.

No violations were noted during the inspection.

2821156 Primary Facility 2019-01-02 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Cash Dollar well pad and the following was observed.

Proper signs were displayed identifying the site. The access road was maintained and in good condition. The outer slopes were stabilized with thick vegetation no erosion observed outside of the LOD. The well heads were maintained no leaks or defects observed. The production tanks were in good condition no defects or leaks.The production tanks secondary containment was intact.


Note: Wells 125-28214 and 125- 28156 violations for no secondary containment under the GPU's 78a.64a(a). The other wells on the pad were spudded and drilled prior to the 2016 regulation and are exempt of 78a.64a(a).
