The access road has been constructed to the pad location; the pad area has been cleared and partially grubbed. At the time of my inspection, the grinding/shredding machine being used to prepare the site and generate fill material for the silt sock BMPs was broken down, and so not all BMPs had been installed. The site foreman indicated that once the machine was repaired, the remainder of the perimeter E&S BMPs would be installed. No evidence of accelerated erosion or sedimentation was observed. Grading work on the well pad location was underway, and two infiltration basins are nearly complete. Additional stone was being added to the access road. Temporary erosion and sediment control BMPs are in place in accordance with the approved E&S plan. No evidence of accelerated erosion or sedimentation was observed. Mr. Wade indicated that seeding and mulching would begin along the access road within the next day or two. During the inspection a fuel truck arrived to fill the equipment operating on site. It was noted that no containment was in use while equipment was being filled. I informed Mr. Wade that containment is necessary during re-fueling activities to minmize the potential of pollution to waters of the Commonwealth. Construction of the well pad, access road and stormwater infiltration basins is complete. At the time of inspection, crews were applying seed and straw to the disturbed areas of the site. No evidence of accelerated erosion or sedimentation was observed. Erosion control matting installed in the ditch along Hemlock Road adjacent to the well site access road was not properly installed. The matting was not keyed in along the edges as recommended in the Department's Erosion and Sediment Pollution Control Probram Manual. In addition, some areas of matting along the access road ditches were not keyed in. It is recommended that the matting be keyed in to prevent movement during flow events within the ditches in order to function properly until vegetation is established. No violations were cited during this inspection. |

Earth moving activities are complete. Some minor earthwork was underway on a rock lined ditch along the perimeter of the well pad. The erosion control matting in the roadside ditches has been keyed in as recommended in the 07/27/2016 inspection report. Some minor areas of erosion resulted from the heavy rains that occurred last weekend. These areas consisted of small rills that developed in recently seeded and mulched portions of the limit-of-disturbance (LOD). None of the eroded material left the LOD. No other areas of accelerated erosion or sedimentation were observed. No violations were cited during this inspection. |

I noted a small amount of water with what appeared to be drill mud in it in containment next to the berm near the cuttings backyard; I recommended that it be vacced up. I also noted a small amount of light staining near the chemical storage area that appeared to be oil or drill mud. These spots were small and I recommended they be cleaned. The rig and associated equipment have been moved off location. After the removal of the rig matting and secondary containment a hole was discovered in the polyurethane plastic containment. This hole resulted in oil based drilling mud being released onto the pad surface around the 1H well cellar. An under drain going to the well cellar resulted in the mud being released into a nearby sediment trap. The amount of mud released into the sediment trap is unknown. A thin film can be seen on top of the now dry surface of the sediment trap.

This inspection followed Chief’s notification to the Department earlier in the day of bubbling outside surface casing for the Hemlock Hunting Club B 1H and 2H. Hemlock Hunting Club B 1H: The cellar was dry. There was fluid in the well head flutes above the conductor pipe x surface casing annulus. Bubbles were observed at the big flute (steady bubbling, approximately 50 bubbles/minute) and adjacent two flutes (intermittent). The bubbling was verified as combustible gas (15% LEL) using an Altair 5X gas meter (calibrated 6/27/17), tubing, and a collection cup positioned at the wellhead flute (collection cap over bubbles, but not well located due to limited access). It appeared that PCSM had been constructed and at the time of inspection was filled with ice/snow (Photos #1, 2). The site was snow covered preventing the assessment of vegetative cover and E&S issues. The result of this inspection is marked as Outstanding Violations due to those cited in the Administrative Inspection from 7/11/17, ID#2614132. No new violations noted. |
