The access road has been constructed to the pad location; the pad area has been cleared and partially grubbed. Temporary erosion and sediment (E&S) control BMPs are in place along the access road, and a portion of those at the well pad location have been installed. At the time of my inspection, the grinding/shredding machine being used to prepare the site and generate fill material for the silt sock BMPs was broken down, and so not all BMPs had been installed. The site foreman indicated that once the machine was repaired, the remainder of the perimeter E&S BMPs would be installed. No evidence of accelerated erosion or sedimentation was observed.

No violations were cited during this inspection.

2498609 Primary Facility 2016-07-05 Routine/Complete Inspection No Violations Noted Routine inspection to assess site conditions during construction phase activities. Merle Wade of Chief was present during the inspection.

Grading work on the well pad location was underway, and two infiltration basins are nearly complete. Additional stone was being added to the access road.

Temporary erosion and sediment control BMPs are in place in accordance with the approved E&S plan. No evidence of accelerated erosion or sedimentation was observed. Mr. Wade indicated that seeding and mulching would begin along the access road within the next day or two.

During the inspection a fuel truck arrived to fill the equipment operating on site. It was noted that no containment was in use while equipment was being filled. I informed Mr. Wade that containment is necessary during re-fueling activities to minmize the potential of pollution to waters of the Commonwealth. Chief should ensure that all contractors on site are familiar with the PPC plan for the site in order to reduce the potential for pollution inc

2505053 Primary Facility 2016-07-27 Routine/Complete Inspection No Violations Noted Routine inspection to assess progress of pad construction. No Chief representatives were present.

Construction of the well pad, access road and stormwater infiltration basins is complete. At the time of inspection, crews were applying seed and straw to the disturbed areas of the site. No evidence of accelerated erosion or sedimentation was observed.

Erosion control matting installed in the ditch along Hemlock Road adjacent to the well site access road was not properly installed. The matting was not keyed in along the edges as recommended in the Department's Erosion and Sediment Pollution Control Probram Manual. In addition, some areas of matting along the access road ditches were not keyed in. It is recommended that the matting be keyed in to prevent movement during flow events within the ditches in order to function properly until vegetation is established.

No violations were cited during this inspection.

2508188 Primary Facility 2016-08-05 Routine/Complete Inspection No Violations Noted Routine inspection to assess progress of pad construction. Merle Wade of Chief was present.

Earth moving activities are complete. Some minor earthwork was underway on a rock lined ditch along the perimeter of the well pad. Temporary stabilization and seeding of the site has been completed and germination is evident in some areas. The erosion control matting in the roadside ditches has been keyed in as recommended in the 07/27/2016 inspection report.

Some minor areas of erosion resulted from the heavy rains that occurred last weekend. These areas consisted of small rills that developed in recently seeded and mulched portions of the limit-of-disturbance (LOD). None of the eroded material left the LOD. No other areas of accelerated erosion or sedimentation were observed.

No violations were cited during this inspection.

2582096 Primary Facility 2017-04-11 Routine/Complete Inspection No Violations Noted Two wells spud with conductor. Several waste containers on the site, on which there was some rubbish. Crew from Mr Dirt were on location during my inspection.
2597410 Primary Facility 2017-05-24 Routine/Complete Inspection No Violations Noted I arrived on-site @ 10:50 hrs. and met Jeremy Boyd and Jeremy Watson of Chief. Precision rig 569 on location drilling the B-1H, currently @ 8550'. Sitewide liner was mostly covered by rig mats, but the berms and drive-over areas were in good condition. I noted a small amount of water with what appeared to be drill mud in it in containment next to the berm near the cuttings backyard; I recommended that it be vacced up. I also noted a small amount of light staining near the chemical storage area that appeared to be oil or drill mud. These spots were small and I recommended they be cleaned. I departed the site @ 11:40 hrs.
2599053 Primary Facility 2017-05-26 Drilling/Alteration No Violations Noted The B 1H well was being drilled. The rig (Precision Drilling 569) was at down hole depth of 13,500 feet. A small amount of mud escaped from a hopper and was being vacuumed up within containment. No leaks detected from other equipment. Secondary containment was sound and in good condition. Some E&S concerns were noted and are to be addressed by the operator. No violations noted.

2610152 Primary Facility 2017-06-30 Incident- Response to Accident or Event Violation(s) Noted The 1H and 2H wells were recently drilled. The rig and associated equipment have been moved off location. After the removal of the rig matting and secondary containment a hole was discovered in the polyurethane plastic containment. This hole resulted in oil based drilling mud being released onto the pad surface around the 1H well cellar. An under drain going to the well cellar resulted in the mud being released into a nearby sediment trap. The amount of mud released into the sediment trap is unknown. A thin film can be seen on top of the now dry surface of the sediment trap.

An "L" shaped trench measuring 84 by 45 feet and 3-4 feet deep was dug from the well cellar. This trench was dug along the underlain drain line.

2618984 Primary Facility 2017-06-30 Follow-up Inspection Pending Arrived @ 10:30. Weather: hot, humid. There were 2 completed, but not stimulated, wells on site. This inspection followed Chief’s notification to the Department earlier in the day of bubbling outside surface casing for the Hemlock Hunting Club B 1H and 2H. Kellyn Balum (Chief environmental) and Mr Dirt workers were addressing a release of Synthetic Oil-Based Mud (SOBM) at the time of my inspection.

Hemlock Hunting Club B 1H: The cellar was dry. There was fluid in the well head flutes above the conductor pipe x surface casing annulus. Bubbles were observed at the big flute (steady bubbling, approximately 50 bubbles/minute) and adjacent two flutes (intermittent). The bubbling was verified as combustible gas (15% LEL) using an Altair 5X gas meter (calibrated 6/27/17), tubing, and a collection cup positioned at the wellhead flute (collection cap over bubbles, but not well located due to limited access). The annulus vents (13-3/8” X 9-5/8” and 9-5/8” X 5-1/2”) were shut at the time of my inspection. A follow-

2619017 Primary Facility 2017-07-05 Follow-up Inspection Violation(s) Noted Weather: Sunny and beautiful. Met with John Hafer (Chief Environmental Specialist) at 9:30 to further evaluate methane bubbling at the Hemlock Hunting Club B 1H and 2H wells (previously described on 6/30/17, IR # 2618984). The 1H and 2H are the only wells onsite. Previous observations of bubbling outside surface casing were made when the annular vents were shut-in. For this inspection, the annular vents on both wells were open to a polytank. Water and vac trucks were also onsite so that water levels could be varied to further evaluate the presence of stray gas inside the cellars of both wells.

Hemlock Hunting Club B 1H: bubbling was observed outside, but adjacent to, the conductor pipe. The bubbling was verified as combustible gas (15% LEL) using an Altair 5X gas meter (calibrated: 7/5/17), tubing, and a collection cup positioned above the bubble location. The bubbling at the surface casing x conductor pipe annulus was also observed during this inspection, but the rate of bubbling appeared to be somewhat l

2619050 Primary Facility 2017-07-11 Follow-up Inspection Outstanding Violations - Viols Req'd Weather: cloudy, following rain, warm. Caleb Woolever (DEP Geologic Specialist) and I met with John Hafer (Chief Environmental Specialist) at 9:00 to attempt to collect isotopic gas samples of methane bubbling outside of the conductors on Hemlock Hunt Club B 1H and 2H wells (previously described on 7/5/17, IR # 2619017). The 1H and 2H are the only wells onsite. Also on location were welders preparing to install conductor caps on the wells. Previous observations of bubbling outside conductor were made when the annular vents were open, but had been open for less than 48 hours. For this inspection, the annular vents on both wells were open to a polytank and had been open for an additional 6 days. Water and vac trucks were also onsite so that cellar water levels could be varied.

Hemlock Hunt Club B 1H: sporadic bubbling was observed outside, but adjacent to, the conductor. For the Hemlock Hunt Club B 1H, there was not sufficient gas expression to acquire an isotopic sample at the time of the inspection. Left

2624755 Primary Facility 2017-08-03 Follow-up Inspection Outstanding Violations - Viols Req'd Weather: cloudy, after rain. Arrived @ 14:30 with Chief Environmental Specialist Jon Hafer to conclude 7-day PBTs on the Hemlock Hunt Club B 1H and 2H, prior to the wells being fracced. Both cellars contained water and bubbling was present in both cellars/ Pressures at the conclusion of the test were as follows:

Hemlock Hunt Club B 1H: 9-5/8” X 5-1/2”: 561.8 psig @ 15:00; 13-3/8” x 9-5/8”: 170.20 psig @ 15:05, 

20” x 13-3/8”: 17.15 psig @ 15:05.

Hemlock Hunt Club B 2H: 9-5/8” X 5-1/2”: 766.25 psig @ 14:50; 13-3/8” x 9-5/8”: 81.85 psig @ 14:55, 20” x 13-3/8”: 12.10 psig @ 14:55. Hemlock Hunt Club B 1H: Prior to collecting split samples for gas outside of conductor on the Hemlock Hunt Club B 2H, we needed to wait for a water truck to remove water from the cellar. The water truck arrived at 15:30. While the truck was removing water from the cellar of the Hemlock Hunt Club B 2H, Mr Hafer and I split isotopic samples for gas outside of conductor on the Hemlock Hunt Club B 1H. 2624868 Primary Facility 2017-08-07 Follow-up Inspection Outstanding Violations - Viols Req'd Weather: rain. Arrived @ 8:30 to meet Chief Environmental Specialist Jon Hafer to observe isotopic sample collection from annuli on the Hemlock Hunt Club B 1H and 2H. Met with Justin Myers, company man on the frac. At the time of my inspection, crews from Universal and Renegade were preparing for operations. Prior to my inspection, the 9-5/8” x 5-1/2” annuli of both wells had been pumped out, isolated and purged, then shut in at approximately 18:00 the previous day. A pressure transducer was attached to he 9-5/8” x 5-1/2” vent which was also plumbed to the gas buster in prep for the frac. During operations the 9-5/8” x 5-1/2” will be open, but if flow were to substantially increase, the transducer would alarm in the frac trailer. During the Hemlock Hunt Club B 1H and 2H fracs, all the annuli will be open and venting. All annuli on both wells had sufficient gas for isotopic sample acquisition.
2643811 Primary Facility 2017-10-04 Routine/Complete Inspection Outstanding Violations - Viols Req'd Arrived at 12:52. Weather: sunny, nice, nice breeze. Encompass crew on site hydrotesting pipeline. Backer tanks (32) on one side of pad positioned on containment. 3 baker tanks also near wells. There are 2 wells on site, awaiting pipeline. Liner still down on pad near wells. HHC B 1H and HHC B2H wells plimbed to same 550-gal poly tank, contained 100 gal SOBM-water mix. HHC B2H plumbing gurgling.100%/volume methane at polytank opening (Altair 5X gas meter calibrated 10/2/17). There are outstanding violations associated with these wells. No new violations were observed during this inspection. The result of this inspection is OUTST.

2836769 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:41; the weather was ~45°F, clear, and breezy. The gate was open when I arrived.

There were 2 properly tagged Marcellus wells staged for production. The B-1H cellar contained water and no bubbling was observed at the time of this inspection. The B-2H cellar was filled with ice. The wells were plumbed to a poly vent tank that was not on additional secondary containment. There were 2 GPUs and produced water tanks onsite and in secondary containment.

It appeared that PCSM had been constructed and at the time of inspection was filled with ice/snow (Photos #1, 2). The site was snow covered preventing the assessment of vegetative cover and E&S issues.

The result of this inspection is marked as Outstanding Violations due to those cited in the Administrative Inspection from 7/11/17, ID#2614132.
