2543574 Primary Facility 2016-12-06 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 13:00p.m., I met with company man Taylor Jeski, SDC Rig 45 was on site to drill the production hole for the lateral section of the well. The top hole section of the well was drilled and completed on 11/6/12. The rig ran in the hole to circulate the drilling mud out of the hole that was put in by the top hole rig. There is no mouse hole, so the rig pulled the drill pipe out of the hole to make a stand, they were running back into the hole with single joints at time of inspection. I took a walk around the location and there were no obvious concerns noticed.

Depth at time of inspection was 5,610'. Projected TD for the well is 13,180'. I left the site at 13:20p.m.

2618611 Primary Facility 2017-07-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:05a.m.

The well was recently completed and is in production. The well tag was on the well and correct. There was containment around the well, there was also two 500bbl tanks on containment that were empty, the containment appeared to be in good condition. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 727psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:45a.m.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2768513 Primary Facility 2018-08-29 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
