2058782 | Primary Facility | 2012-04-09 | Drilling/Alteration | No Violations Noted | At time of inspection - moving Falcon Rig 17 on location. I met Company Man Al Simondi. I walked site. Secondary containment is in place. Permit is not posted yet. Informed Co. man that permit needs to be posted prior to drilling and site ID sign needs to be updated with proper permit information. Co. Man printed permit while I was on site. Plan to commence drilling late tomorrow (4/10/12). |
2059483 | Primary Facility | 2012-04-13 | Drilling/Alteration | No Violations Noted | Falcon 17. Co. man - Bud Woosley. Had to change rig out - blew motor on other rig. At time of inspection Schlumberger on site preparing to cement 13 3/8". Secondary containment in place. Checked cellar - no bubbling observed. Noticed while inspecting plastic - numerous dimples/pressure points from rocks on pad surface. Recommend Operator use a finer surface material and flat roll pad prior to laying plastic to prevent holes in containment. |
2066879 | Primary Facility | 2012-05-09 | Drilling/Alteration | No Violations Noted | DDI-10. Just moved in to location. Testing BOP's. Currently having trouble with leaks on BOPs. Crews on site installing berms on containment. Observed numerous tears in plastic. Plastic contractos on site installing plastic and repairing containment at time of inspection. Drillers log on site and up to date. No violations observed at this time. |

2540085 | Primary Facility | 2016-11-22 | Routine/Complete Inspection | No Violations Noted | On 11/22/16, the Department conducted an inspection of the SWN RU 18 Lyncott Corp Pad as part of an investigation into a complaint that a nearby water supply may have been affected by natural gas drilling operations. During the inspection it appeared as if all 7 wells (1H, 2H, 3H, 4H, 5H, 6H, and 7H) present were in production. The last well drilled and stimulated on this site is the 7H with drilling having been completed on 2/3/14 and stimulation having been completed on 5/31/14. In general, the pad appeared to be stable with no violations noted. |
2576724 | Primary Facility | 2017-03-27 | Routine/Complete Inspection | No Violations Noted | At time of inspection, arrived on site at 10:20a.m.
The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 674psi on the production casing and 570psi on the production line. There was a 500bbl flow back tank on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:15a.m. |
2674579 | Primary Facility | 2017-12-12 | Routine/Complete Inspection | No Violations Noted | At time of inspection.
