I took a walk around the location, everything looked OK.

The last E&S inspection was performed on 3/7/2016. The wells associated with this site are as follows:

There were no erosional issues with the site, all slopes were stable and there were no failures of BMPs to note. There was however, a large spoil pile on the site, probably in excess of 100 tons. This spoil pile was mostly rock, and was not eroding or sending sediment off site but was not mulched, and it would be suggested that a SWN representative assess this pile for future concerns.

2488706 Primary Facility 2016-05-03 Routine/Complete Inspection No Violations Noted The Department was on site to inspect the former WPX (Now SWN) site of the Moore Well Pad (WR45)(ESX12-115-0078). The wells associated with this site are as follows:

There were no erosional issues with the site, all slopes were stable and there were no failures of BMPs to note. There was however, a large spoil pile on the site, probably in excess of 100 tons.

2508240 Primary Facility 2016-08-04 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:40p.m., the cellar was full of gravel and no gas was detected. The vent pipe was open and set up with a Tayler plug to check for pressure, there was 185psi on the production casing and 145psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 14:05p.m.

2542144 Primary Facility 2016-12-01 Incident- Response to Accident or Event Pending Michael Williams, Evan Brandt and Chris Becker, Environmental Health and Safety Specialist of SWN contacted me at 4:07 PM on 11/30/16 to report a release of gas discovered at 2:30 PM on 11/30/16. They stated that a subsurface production fluid dump line failed creating the potential for an impact to the pad in the general area where the dump line is located. Mr. Brandt forwarded me an email at 5:11 PM on 11/30/16. The email stated the following. "Per our conversation earlier, a suspected buried dump line (area between the wellhead and production tank) failure occurred on the WR-45-Moore Pad earlier this afternoon. At this time, we are unaware of any environmental impacts outside of the dump line ditch. SWN Regulatory has been continually testing the surface throughout the afternoon and has found no elevated conductivity levels. All wells on location are shut in and we will begin investigating the subsurface area tomorrow morning 12/1/2016 at 9:00 am. Resource Environmental has been contracted to perform
2546885 Primary Facility 2016-12-21 Administrative/File Review Violation(s) Noted On December 1, 2016, the Department and SWN Production Company, LLC (Southwestern) collected groundwater samples from the excavation at the SWNPC Moore Pad. Upon review of the Department’s and SWNPC's groundwater sample results, it was noted that the sample results had elevated concentrations of barium, strontium, aluminum, chloride, iron and manganese. Barium was detected above the Act 2, Residential and Non-residential Medium Specific Concentration (MSC) for groundwater of 2,000 micrograms per liter (µg/l) with a concentration of 33,570 µg/l detected in the Department’s sample and 34,500 µg/l detected in SWNPC's sample. Strontium was detected above the Act 2, Residential and Non-residential MSC for groundwater of 4,000 µg/l with a concentration of 52,200 µg/l detected in the Department’s sample and 48,800 µg/l detected in SWNPC's sample. Aluminum was detected above the Act 2 Secondary Maximum Contaminant Level (SMCL) of 200 µg/l with a concentration of 9,581 µg/l detected in the Department’s sample and
2548815 Primary Facility 2017-01-03 Follow-up Inspection No Violations Noted The Department, represented by Frank Hoehle, was on site to conduct a Follow Up Inspection of the Moore well site (West Range Unit 45) (ESX12-115-0078). This well site had a release occur on 12/1/2016 with subsequent sampling events and NOV occurring.

According to Mike Deming, SWN representative, screening and vac truck excavation around suspected leak area ended Thursday, December 8, 2016 and confirmatory sampling for this specific area occurred Friday December 9, 2016. Upon excavation, elevated conductivity readings were observed flowing into excavation from West side of brine tank area along back of pad. SWN will be sampling homes in the vicinity, one of which has elevated conductivity historically. SWN now digging test pits on East side of pad to check for possible contamination left over from previous ownership.

There were no further violations.

2602041 Primary Facility 2017-06-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:45p.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 310psi on the production casing and 116psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:10a.m.

2661957 Primary Facility 2017-11-15 Routine/Complete Inspection No Violations Noted I inspected the Moore well pad on November 15 as a Routine Complete inspection. The well pad was solid and had no current erosional issues, and was well vegetated. There had been a produced water release here approximately a year ago. The cut and fill slopes were solid and stable as was the access road.

There were no violations noted.

2706401 Primary Facility 2018-03-16 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and covered with snow, the snow insulated the gravel so it wasn't frozen and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2793938 Primary Facility 2018-10-24 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with Cody Stanton (Production Foreman) for SWN. The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. Cody used a Crystal pressure gauge to get the pressures for the ((9x5) annulus, production casing and tubing. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2814754 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Moore well pad. The weather was overcast and about 32 degrees F. There was a light covering of snow on the ground and there was construction work being performed along the access road and the fill slope. There was stone staged near the entrance to the access road.
    An infiltration basin PCSM was apparently being constructed.  There were competent temporary BMPs downgradient of the ongoing construction work.
    The cut slope was mostly rock and appeared stable.  There was a smattering of vegetation visible where suffient soil was available to support growth.  The fill slope, while under construction in part, was well vegetated otherwise.  The vegetation appeared to be a mix of perennial grasses.
   The well pad was snowcovered, solid and stable.  There was a large spoils pile on site but it was not causing any problems regarding erosion and sedimentation.
    The well onsite was labeled properly.
    There were no violations noted.
