2241311 Primary Facility 2014-01-29 Incident- Response to Accident or Event Pending On 1/29/14 at approximately 12:00 PM, Afton Sterling contacted the Department and stated that a landowner had identified a water leak coming from a ball valve located on a pump assist site for the water line on the ROW of the Susquehanna Gathering South System Well Pad 39 to Bluestone Pipeline (ESX12-115-0166). SWN has assumed responsibility for the incident. The leak is approximately 1000’ from the SWN RU24 Woosman Pad. Afton stated that the spill was approximately 5 gallons, that it appeared not to have contacted the ground due to the water freezing and turning to ice, and that field tests of pieces of the ice having been melted revealed conductivity less than 1000 µS.
2246609 Primary Facility 2014-02-20 Administrative/File Review No Violations Noted On 2/20/14, SWN submitted to the Department the results of the sampling that they had conducted for the above mentioned spill. A review of the sample results finds that the spilled water is not contaminated and that the resulting ice may stay in place with no further action required by the Department.

The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 495psi on the production casing and 365psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

2753536 Primary Facility 2018-07-16 Incident- Response to Accident or Event Pending I performed an incident inspection in conjunction with Jon Sassi at the Woosman well pad. The wells associated with this pad are the 115-21054, 115-22377, 115-22402, 115-22378, 115-22403, 115-22379. The well site was being fracked at the time of inspection, The company man was Mark McPherson. The incident occurred July 13, 2018 at 2:45 p.m. when a mixture of production water and freshwater was released. The estimated release was not known. The containment was inspected in the release area and no patches tears or holes were noted. The material in the immediate area was excavated upon discovery but more high conductivity water keeps infiltrating the sump area. The incident seems to be traced back to the blender and associated piping. The piping for the area is undr 120psi pressure. The equipment is scheduled to be moved off the containment by Wednesday and the site can then be assessed further. The one operating well on site is shut in during frack operations.

2753546 Primary Facility 2018-07-16 Routine/Complete Inspection No Violations Noted I performed an RTNC inspection in conjunction with Jon Sassi at the Woosman well pad. The wells associated with this pad are the 115-21054, 115-22377, 115-22402, 115-22378, 115-22403, 115-22379. The well site was being fracked at the time of inspection, The company man was Mark McPherson.

The site had good growth and was solid and stable at the time of inspection.

There were no erosional issues noted with the site at the time of inspection.

2760405 Primary Facility 2018-07-20 Follow-up Inspection No Violations Noted The Department was on site to conduct a follow up investigation of a spill that occurred on 7/13/2018. Present representing the Department were Frank Hoehle (WQS) and myself. We arrived on site at 1215 hours.

We talked to Donnie Torres (Well Site Supervisor-SWN). He informed us that crews were setting up on site to perform drill out and flowback operations. The one producing well on the pad was shut in. The others have been fully drilled and stimulated. Drill out operations on the plugs had not yet began.

The release occurred on the southwest corner of the pad, when a blender and its associated piping failed, causing a release of production fluid and freshwater in containment. The estimated release volume is not known. Material in the immediate area under the containment was screening high for conductivity. The material was excavated, but more high-conductivity water kept infiltrating the sump area below the containment. At the time of the initial response inspection, it did not appear as if there were a

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. There were twenty 500bbl frac tanks on containment from a recent frac. Most of the tanks were empty and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
