2256746 Primary Facility 2014-03-11 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 13:10 in response to a spill of (5) barrels of flowback water to containment. Southwestern Energy reported the spill to the Department at 8:45 AM on 3/12/14. The Marcellus incident report indicates the spill occurred at 6:45 AM on 3/12/14 and (5) barrels of flowback water spilled onto containment. It was reported that no material came into contact with the ground. Seth Tratthen, Ryan Klemish and Mike Tornes of SWN accompanied me during the inspection. Mercer Rig #127 is the active rig on location in the process of installing production tubing. Mr. Tornes informed me the spill occurred while SWN was in the process of drilling out frac plugs. Flowback water overflowed out of a hatch on the top of frac tank number (26927) in the the tank battery, onto containment. The spill occurred as a result of operator error according to Mr. Tornes. Mr. Tratthen stated the spill occurred at 12:05 AM on 3/12/14.
2353569 Primary Facility 2015-03-20 Routine/Complete Inspection No Violations Noted Inspection of the Mulloy 1H on March 20th 2015 at 2pm. Well was in production at time of inspection. There was 802psi on production casing as indicated on gauge above wing valve. Gauge on production line out read 536 psi, valve open to production unit which showed 510psi at the unit itself. Cellar was filled with gravel. One vent line plumbed to surface, shut in and tied into production line out. No signs of leaks or reasons for concern. Proper site id was present, permit number, and cement barriers around well.
2398376 Primary Facility 2015-08-04 Incident- Response to Accident or Event No Violations Noted Falcon Drilling Rig #25 is in the process of drilling the Mulloy 3H well at a depth of (1,644) feet. Chris Becker of Southwestern Energy contacted the Department at 10:30 AM on 8/3/15 to report that at approximately 9:30 AM on 8/3/15, a spill had occurred on the Range Unit 63; Mulloy Pad. The spill occurred while drilling of the 3H well was in progress. While drilling on air, the solids separation unit had a tank that overflowed (38) barrels of freshwater mixed with a polyacrylamide based flocculent. Due to compromised liner integrity in the containment, approximately (1) barrel of the fluid was released to the pad surface.

This report has been generated in response to remediation efforts conducted by SWN in response to a spill incident that had occurred at the site first documented on 8/4/15 by the Department in inspection record # 2398386. The Department concludes that the remediation of the spill conducted by SWN was adequate and that no further action is required of SWN in reference to this spill.

2535958 Primary Facility 2016-11-01 Incident- Response to Accident or Event No Violations Noted On 11/1/16 at 10:55AM, the Department arrived on site. The Department met with Dan Hessdorfer (Well Site Supervisor). Mr. Hessdorfer explained that he was present during the release which occurred at 2:30 PM. Mr. Hessdorfer stated that rainwater was in the containment and that there were four (4) spots that haven’t been properly welded. He also stated that the water appeared clear and that the containment was clean at that time. SWN was drilling the 2H well and waiting on cement at the time of the release. During the inspection, the Department noted that the SDC45 rig was on site and was drilling the 3H well. We walked to the area of the release and found that there was orange spray painted arrows pointing to the breeches in containment. Mr. Hessdorfer explained that they were already patched. The Department screened water and soil in the vicinity (41¿ 51’ 11” -75¿ 36’ 27”) of the release and the readings were 0.20 mS and 0.014 mS respectively.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 656psi on the production casing and 499psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:50p.m.
